SUBJECT: NRC INSPECTION REPORT 50-440/97006(DRS) AND NOTICE OF VIOLATION
. Dear Mr. Myers:
On April 24,1997, the NRC completed a routine safety inspection at your Perry 1 reactor
facility. The inspection involved a review of Perry's maintenance department fuse control
process, procedure / instruction change process, and the selection of qualified maintenance
personnel to do safety-related work. The enclosed report presents the results of that
inspection.
A violation of NRC requirements was identified involving two surveillance instructions that
were performed with uncorrected, erroneous information in the procedure. Non-intent
conditional procedure / instruction changes were initiated, but were not reviewed and
approved prior to test perforrnance. We are concerned that the instrumentation and
control supervisor and the unit supervisor did not follow the instruction change process
prior to performing two surveillance instructions.
This violation is cited in the enclosed Notice of Violation (Notice), and the circumstances
surrounding the violation are described in detrJ fn the enclosed report. Please note that
you are required to respond to this letter and snould follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
