I am retiring from the faculty of Lebanon Valley College, and we therefore desire to amend item 12 of the license by replacing my name with the name Barry L. Ilurst. Ph.D.

Dr. Ilurst has been teaching General Physics, Experimental Physics and Atomic and Nucicar Physics at Lebanon Valley College, and for the last two years has directed laboratory work in atomic and nuc1 car physics including the use of the scaled plutonium-berylium sources in neutron activation and radiation analysis under the supervision of Dr. Rhodes. lie is familiar with the procedures and regulations regarding the use of these licensed sources.

In the absence of a certificate from a transferor indicating that a test has @

(2) The periodic leak test required by this condition does not apply to sealed b;
