NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 As a result of the inspection conducted on October 22 through November 27, 1985, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2 Appendix C (1985), the following violation was identified:
10 CFR 50, Appendix B, Criterion III, as implemented by Illinois Power Company Construction Quality Assurance M.inual, Chapter 3, requires that design reviews and verification measures be establithed and conducted to assure the adequacy and confomance of equipment and processes with design bases, regulatory requirements, and applicable codes and standards.
Contrary to the above, the safety-related mechanical snubber rear bracket of Sargent and Lundy (S&L) designed pipe support 1RH-160355 was welded to the yoke of Seismic Category I, ASME Code Class 2, Residual Heat Removal System, Anchor Darling (A/D) Limitorque valve 1E1Z-F064A, without either the Architect /
Engineer (S&L) or the valve manufacturer (A/D) conducting an engineering evaluation of the valve assembly seismic qualification report to justify the addition of the attachments to the " active" mechanical component.
This is a Severity Level IV violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1)correctiveaction taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
