.p APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No.
50-245 Millstone Point Nuclear Power Station, Unit 1 License No. DPR-21 As a result of the inspection conducted on November 5-8, 1985, and in accord-ance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following vio-lation was identified:
Technical Specification 6.11 requires that procedures for radiation pro-tection be established, implemented and adhered to.
Procedure SHP 4912, Revision 6, Section 4 requires, in part, that workers be responsible for
... reading, understanding, initialing, and following RWP instructions."
Contrary to the above, on October 27, 1985, two maintenance workers entered a radiological area in violation of the requirements listed on the controlling RWP #3344.
Specifically, the workers performed work in the reactor cavity without wearing respiratory protection equipment as required by the RWP. One individual of the working party consequently sustained a limited intake of radioactive material.
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
