o APPENDIX A Transamerica Delaval, Incorporated Engine and Compressor Division Docket No. 99900334/85-01 NOTICE OF NONCONFORMANCE Based on the results of the NRC inspection conducted on June 3-7,1985 it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Section 2.4.1 of the TDI In Process Inspection Manual, dated 5/1/85 reads, in part, "The area inspector will dimensionally and visually inspect the results of the current manufacturing operation in relation to previous and subsequent operations as outlined on the routing sheet using the drawings and other specifications as his criteria. If the part is acceptable at this operation, the inspector will stamp or initial and date the Production Routing Sheet, Form P-135B, in the space provided."
Contrary to the above, operation #95, " Layout and side tap holes," for Part
- 02-375-09-AQ (intake manifold) on Job No.85-330 was neither stamped nor dated by a QC inspector to show acceptance of this operation. However, subsequent operations (i.e., #100, drill and tap flanges; #110, deburr (complete); #120, store) were stamped, signed and dated by various inspectors. In addition, final inspection and acceptance was properly stamped, signed and dated by a QC Inspector.
