I APPENDIX A NOTICE OF VIOLATION Alloy Surfaces Company, Inc. Docket No. 99990001 Wilmington, Delaware License No. General License As a result of the inspection conducted on June 17, 1985, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations l were identified:

A. 10 CFR 31.5(c)(3) states that the tests required by paragraph (c)(2) of this section and other testing, installation, servicing and removal from installation involving the radioactive materials, its shielding or containment be performed either in accordance with the instructions provided by the label or by a person holding a specific license pursuant to Parts 30 and 32 of this chapter or from an Agreement State to perform such activities.

Contrary to the above, licensed material was removed from the licensee's previous location and installed at their present location by a person not specifically licensed to do so.

o This is a Severity Level IV violation (Supplement VI).

Pursuant to the provisions of 10 CFR 2.201, Alloy Surfaces Company, Inc.

is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
