l VOGTLE ELECTRIC GENERATING PLANT-UNITS 1 & 2 REPLY TO A NOTICE OF VIOLATION NRC INSPECTION REPORTS 50-424: 425/96-12 VIOLATION 50-424: 425/96-12-02 l

The following is a transcription of the violation as cited in the Notice of Violation (NOV).

i "During an NRC inspection conducted November 10,1996 through December 21,1996, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," (NUREG-1600), the violation is listed j

below:

j Title Ten Code of Federal Regulations Part 50, Appendix B, Criterion XVI, requires in part, measures be established to assure that nonconformances are promptly identified and corrected. Request for engineering assistance (REA) 96-VAA639, dated August 7,1996, indicated that all Motor Control Center (MCC) panel door latches must be properly secured for the MCC to be within the envelope ofits seismic qualification.

Contrary to the above, on December 5,1996, adequate measures to assure that nonconformances were promptly identified and corrected were not established as evidenced by an inspection of safety related IE 480 volt MCC Nos 2-IS05-S3-2BBA and 2-1805-S3-2ABA, that revealed seven latches not secured on MCC 2 1805-S3-2ABA and 34 latches not secured on MCC 2-1805-S3-2BBA.

This is a Sevenity Level IV violation (Supplement 1)."

RESPONSE TO VIOLATION (50-424: 425/96-12-02)

Admission or Denial of the Violation:

This violation occurred as stated in the notice of violation.

Reason for the Violation:

During a previous NRC inspection, report number 50-424, 425/96-09, the NRC identified MCC panel door latches that were not secured. An engineering evaluation was conducted to determine the impact the unsecured latches would have on the seismic qualification of the MCC. The evaluation which was completed on August 7,1996, indicated that no data had been found to conclude that unlatched or partially latched doors are acceptable. The evaluation also indicated that during the time door latches were broken, a temporary repair for securing the door would be duct tape.

ENCLOSURE VOGTLE ELECTRIC GENERATING PLANT-UNITS 1 & 2 REPLY TO NOTICE OF VIOLATION NRC INSPECTION REPORTS 50-424: 425/96-12 As a result of the evaluation, operations personnel conducted walkdowns to ensure latches on other MCCs were secured. However, on December 5,1996, the NRC identified unsecured latches on MCC Nos 2 1805-S3-2BBA and 2-1805-S3-2ABA.

The reason for this violation was a failure to clearly communicate to plant personnel the results of the evaluation concerning the importance for ensuring that latches are properly secured and that broken latches can be temporarily repaired using duct tape.

Corrective Steps Which Have Been Taken and the Results Achieved:

1.

The unsecured latches on MCC Nos 2-1805-S3-2BBA and 2-1805-S3-2ABA have been secured.

2.

Latches on all other IE MCCs and on all IE switchgears have been inspected.

3.

Operations shift briefings have been conducted to provide clarification and expectations to operators concerning their responsibilities for inspecting MCC door latches during daily rounds. The briefings also described the method of temporarily repairing broken latches and the follow-up actions required to ensure the latches are properly repaired.

Corrective Steps Which Will Be Taken to Avoid Further Violations:

1.

Operations personnel will receive additional training on the importance for ensuring MCC latches are secured. This training will be completed by April 11,1997.

2.

As an interim measure, a Maintenance Department briefing will be conducted to ensure personnel understand that MCC door latches are to be properly secured after maintenance / testing activities. This will be completed by February 18,1997.

3.

Maintenance personnel will receive additional training on the importance for ensuring MCC latches are secured. This training will be completed by April 11,1997.

Date When Full Compliance Will Be Achieved:

Full compliance was achieved on December 8,1996, when the latches on all IE MCCs were secured.
