APPENDIX A NOTICE OF VIOLATION Fairchild Semiconductor Docket No. 99990001 South Portland, Maine 04106 General License As a result of the inspection conducted on August 14, 1985, and in accordance with th'e NRC Enforcement Policy (10 CFR 2, Appendix C), the following violations were identified:
10 CFR 31.5(c)(4) requires that records of tests for leakage of radio-active material required by 10 CFR 31.5(c)(2) be maintained for one year after the next required leak test is performed or until the sealed source is disposed of or transferred.
Contrary to the above, as of August 14, 1985, records of leak tests performed on the Fisher Technology thallium-204 sealed sources (S/N 0837M) were not maintained.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, Fairchild Semiconductor is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the dato when full compliance will be achieved.
