Dear Dr. Murley:

Millstone Nuclear Power Station, Unit No. 3 Reporting of Pctential Significant Deficiencies in Design and Construction:

Nonconforming Terminations in CVI Corporation Equipment (SD-75)

In a February 13, 1985 telephone conversation between your Mr. T. Rebelowski and our Mr. R. K. McCarthy, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e).

As acted in Reference (1), the potential significant deficiency involved improper terminations found in equipment supplied by CVI Corporation.

Termination deficiencies include crimps which were unacceptable per vendor's instructions, terminations crimped with the wrong tools, multiple conductors crimped in one lug, and incorrect lug size used.

In Reference (2) NNECO determined that this condition represented a significant deficiency in the final design of Millstone Unit No. 3.

t NNECO also indicated in Reference (2) that a 100% inspection of the legs in the two remaining CVI panels (3 HVC*PNLFLT 1 A and IB) would be performed. Due to various system testing which is being performed at this time, the inspection has not been completed. As such, NNECO is not able to provide a final report.

NNECO will provide you with an update on this matter by November 8,1985.

Due to administrative delays, this update is being provided on October 9 rather than October 8, as discussed with your Mr. T. Rebelowski.
