OPERATING REACTORS EVENTS BRIEFING APRIL 30, 1997 - BRIEFING 97-03 On April 30, 1997, we conducted an Operating Reactors Events Briefing (9/-07) 4 to inform senior managers from offices of the Commission, AE00, NRR and i
regional offices of selected events that occurred since our last briefing on March 12, 1997. Attachment I lists the attendees. Attachment 2 presents the significant elements of the discussed events.
, contains reactor scram statistics for the weeks ending March 16, March 23, March 30, April 20, and April 27, 1997. No significant events were identified for input into the NRC Performance Indicator Program.
The statements contained in the attached briefing slides represent the best information ' currently available to the NRC.
Future followup could produce new information that may alter the NRC's current view of the events discussed.

A FAULT IN THE MAIN TRANSFORMER PROVIDED A LEAKAGE PATH l
FOR SEVERAL THOUSAND GALLONS OF INSULATING OIL TO ENTER l
THE TURBINE BUILDING.

CAUSE A FAULT IN THE TRANSFORMER RUPTURED A LOW-VOLTAGE BUSHING, PROVIDED A LEAKAGE PATH.
L SAFETY SIGNIFICANCE THE POTENTIAL FOR A SIGNIFICANT FIRE IN THE TURBINE BUILDING THAT COULD HAVE AFFECTED BOTH DIVISIONS OF SAFETY-RELATED SWITCHGEAR.

e AUGMENTED FUEL POOL COOLING (AFPC) MODE OF RESIDUAL HEAT REMOVAL (RHR) WAS LOST WHEN BUS A6 SWAPPED OVER TO THE "B" EDG.
AFPC WAS RE-ESTABLISHED WITHIN 50 MINUTES.
POOL TEMPERATURE ROSE FROM 86 F TO 88 F.
e AN OPERATOR REPORTED OIL SPILLING INSIDE THE TURBINE BUILDING VIA THE ISO-PHASE BUS DUCT AIR COOLER WITHIN MINUTES OF THE TRANSFORMER FAULT.
e 4,300 GALLONS OF OIL SPILLED INTO THE TURBINE BUILDING, SPREADING OUT OVER 2,200 SQ FT ON THE GROUND FLOOR (23-FT ELEVATION), 500 SQ FT OF SWITCHGEAR ROOM "B" (1E SWITCHGEAR), AND 1,350 SQ FT OF THE TRUCKLOCK (LOADING / UNLOADING) AREA.
e THE OIL ALSO SPILLED DOWN A STAIRWELL INTO THE LOWER LEVEL OF THE TURBINE BUILDING, COVERING ABOUT 3,000 SQ FT.
