Dear Dr. McGann:
This letter is in response to your facsimile dated March 20,1997, requesting that registration certificate NR-399-D-101-E be made inactive. In your facsimile, you state that you cannot find any background information on the models identified in this registration certificate or those mentioned in the August 5,1994 letter from Doug Broaddus. However, on the same day of this facsimile, you called Mr. Broaddus and stated that you had found some information and would be sending this information in to him. To date, this information has not been received. Please provide whatever information you can on these devices. Since you state in your facsimile that you do not have a copy of the August 5,1994 letter, a copy of it is enclosed.
In addition, the August 5,1994 letter requests information on models 85 and 110/56A which are listed on your license, but are not listed on a registration certificate. Please provide the information contained in 10 CFR 32.26,32.27, and 32.29 for each of these devices.

August 1, 1993, and subsequent letters, with enclosures thereto, we concluje the Model 97A/ VIXEN is acceptable for licensing purposes in accordance with the conditions of the enclosed registration certificate (NR-0399-D-102-E).

l Please be advised that you must manufacture and distribute the products in accordance with the statements and representations contained in your application, with enclosures thereto, and the information set out in your registration certificate. As a general rule, you must request and obtain an amendment to the certificate before you make changes or modifications to the information submitted to obtain the certificate.
1 Please read over the registration certificate in its entirety and notify us immediately of any errors or omissions.
You are obligated to notify us promptly in writing should you decide to no longer manufacture or offer service support for the products.

The maximum external radiation levels for each of these devices on the surface of the device and at 5 and 30 cm from the surface of the device.
