1. Spec.13-FM-2213, Rev. 20, vendor drawing (P2213-101-4) shows set screw (Iten 220) tack welded to hinge support (Item 007). The set screv keeps the hinge pin (Item 038) moving out of the hinge support (Item 007).
2. Spec.13-FM-2213 Rev. 20. Tendor drawing (P2213-73-5) shows the disc nut pin (Itas,245) to be tack welded to the disc nut (Iten 2303).

1. The set screw (Item 220) is not tight assinst t.he hinge pin (Item 038). The hinge pin may slide through the hinge support and the valve shall become inoperable. The set screw in the present application does not secure the pin adequately.
2. The dise nut pin is not tack welded and may cause the disc nut to disengage.

This condition will cause the valve to becces inoperable.

1. CONDITION DgSCRIPTION Anchor Darling (AD) swing check valves require a set screw (Item 220) to be tack welded to the hinge support (Item 007) to prever t the klage '

pin (Item 038) from becoming disengaged from the hinge supoort.

Additionally, a dise nut pin (Item 245) is required to be tack welded to the disc nut (Item 2308) to provide a positive attachment of the disc. See the attached table for appilcable drawing control log numbers.

Investigation Request II-37 was initiated to perform a site inspection of AD swing check valves for missing tact welds. The following two defects were identified as a result of this investigstloa.

1. In valves 1PAFAV005 and IPAF8V022. the set screw was not tight enough against the hinge pin causing a condition wherela the hinge ple-Is-able to slide _througltthe_htnge_ support. _
2. In valves IPCTAV037. IPCT8V038. IPCTAV016. and 1PCTBV020. the disc nut pin was not tact welded to the disc nut as required.

l II. EVALUATION ,

Red this deficiency remained uncorrected, check valves in various safety related systems including Auxillary Feedwater could have failed, thereby precluding safety related systems from performing essential functions.

The root cause of both the above conditions is attributed to errors during assembly of the valves prior to delivery to the site. Bechtel discussed the above conditions with AD and both agreed that either condition 1 or 2 could make the valve Inoperable. The valve could leak at back flow, disengaged parts could restrict the flow through the valve and/or anali disengaged parts could flow through the systes causing damage to other equipeest.

1. Attached is the list of 40 swing check valves used on Walt 1. All the Unit 1 listed valves are being Inspected and documented om special CIP 592.0. The defective valves will be reworked per AD direction and inspected through NCE's SM-5320. SM-5321. SM-5322 4

and SR-5355.

2. Units 2 and 3 will be inspected and reworked as required by
  • IR-48. Special CIP's 592.0, 606.0 and 607.0 have been lattisted to cover the the defects (loose kinge pin and missing tack weldlag en the dise unit) as identified a Section I of this Dtt and also '

13 NONCONFORMANCE A, copy of Bechtel Deficiency Evaluation Report (DER) 84-54 was received by CPCo from Itichtel Power Corporation. The DER which occurred at Palo Verde Nuclear Generating Station identifies problems with Limitorque motor operated butterfly valves, and the way the motor j operators were wired. The identified motor operators for the butterfly valves, could not be opened electrically, when the valves were closed manually. A preliminary investigation did not identify any Midland Energy Center valves with the combination limitorque operators identified. 1. CONDITION DESCRIPTION Cootainment sump isolation valve 1JSIAW673 could not be opened elec-trically after being manually closed. The valve operates correctly when not closed r.anually. This condition was identified during i preoperational testing and documented on SFR 151-723 and NCR SM-4609.

Valves 1JSIAW674, IJSIBUV675 and 1JSIBW676 are similar valves that' perform the same function as the subject valve. They are 24 inch, limitorque motor operated butterfly valves, manufactured by Posi Seal-International and supplied by Combustion Engineering. These valves are required to open automatically on a recirculation actuation signal (RAS) to provide a suction source for the safety injection pumps. A RAS is initiated on low water level in the refueling water tank during safety injection.

II. EVALUATION

Two factors have been identified as contributing to this deficiency;

1) high packing friction and 2) a design problem in the actor operator.

1.

The first factor, high packing friction, results from modifications made to the valve packing boxes. The original packing configura-tion utilized a lantern ring and asbestos type packing. Based on a project decision that applied to all valves at PVNGS, the lantern

! rings were eliminated and the valves were packed with a grafoil type packing. Although this alone should not cause valve operating failures, it may have contributed to some valve torquing out problems experienced at PVNGS.

2. The second factor deals with a design aspect of the Limitorque ope rators. The operators on these valves (Limitorque l N25C SMB-00-10) use rotors to open and close contacts as

! r for position indication, torque switch bypass, open limit a sed limit. These operators were supplied with the closed position contacts and the torque switch bypass contacts on the same rotor. This rotor is set such that when the valve disc reaches the closed position (zero degrees), the closed limit contacts trip and stop the motor. Since the torque switch l'ypass contacts are on the same rotor they cannot be adjusted independently of the l

closed position contacts so they also trip at zero degrees. In effect, torque switch bypass only occurs from the valve mechanical stop position to the zero degree position as indicated in the sketch below.

After closing the valve electrically, the torque switch allows for enough torque to overcome the opening resistive forces within the valve and operator and the additional packing friction discussed above. Since the torque switch has this capacity, torque switch bypass is not required and the valve operates correctly. However, when the valve is closed manually, the disc is driven past the zero degree position to the mechan-ical stop. Apparently, the torque required to open the valve from this position is greater than from the zero degree position (see sketch). This torque exceeds the capacity of the torque switch so bypass is required.

However, as discussed above, the bypass trips at zero degrees. Therefore, the active torque switch trips the motor on an over-torque condition.

III. REPORTABILITY ASSESSMENT 1

Based on the above, this condition is evaluated as reportable under the requirements of 10CFR50.55(e), since if this condition were to remain uncorrected it could represent a significant safety condition.

This project also has evaluated this condition as reportable under 10CFR21.21(b)(3). This report addresses the reporting requirements of the regulation with the exception of subpart (vi), regarding the number and location of such ecmponents supplied to other facilities.

IV. CORRECTIVE' ACTION ~-~ ~

NCR S.1-4609 has been dispositioned to rework the operators to use a different rotor (spare rotor 4) for the torque bypass. DCPs 1SE, 2SE and 3CE-SI-500 have been issued to complete this work on all four valves per unit identified in Section I and the additional four valves per unit identified in Section II.

No action is required with respect to packing since the above modifica-1 tion will allow enough torque to overcome some excessive packing friction.

A copy of this report will be transmitted to Fosi Seal International and Combustion Engineering.

Again, the notification applies to only those 1153 Series 8 t

transmitters manufactured after January 10, 1984, and those units installed in an environment subject to a LOCA or HEL8.

Currently we are in an extensiva evalu'ation and test program to determine the cause of the leak path. Again, once the i

cause is identified a corrective action will be established.

We expect the results of our testing to be available in approximately two weeks. Another status letter will be sent to you at that time.

traveling screen baskets were removed to facilitate the scope of the work order (ie, remove water and clean out the bays.). A revision to the work order was not deemed necessary at the time since the equipment would be placed back into it's original configuration.
