QUADREX REPORT FOUND THAT BROWN a ROOT APPARENTLY FA! LED TO PROVIDED INCCRRECT INFORMATION TO A EXPERT CONSULTANT, WHO USED PROPERLY IMPLEMENT THE CA PROGRAM IN THE DESIGN AREA SUT ALSO THE INFORMATION IN DEVELOPING THE SE!SMIC RESPONSE SPECTRA FOR FAILED TO PROPERLY IMPLEMENT AN OVERALL DESIGN PROCESS CONSISTENTTHE DESIGN OF CERTAIN SEISMIC P! PING AND EQUIP"ENT RESTRAINTS.
WITH THE NEEDS CF A NUCLEAR PCWER PLANT. AS A RESULT VERIFICA- UR INVESTIGATORS HAVE FOUND THAT THERE WAS A LACK 0F RIGOR AND T!ON OF DESIGN INFCRMATICM WAS APPARENTLY NOT PERFORMED IN A F RMALITY IN THE PROCEDURES USED FOR VERIFYING THE ACCURACY OF TIMELY MANNER, AND REGULATORY COMMITMENTS FOR SAFETY DID NOT INFORMATICN TRANSFERRED BY PGSE TO ITS CCNSULTANTS. THESE APPEAR To BE FULLY OR PROPERLY IMPLEMENTED TO SATISFY NRC REcutRE- PROCEDURES DID MOT CCMPLY WITH OUR REQUIREMENTS CALLING FOR VER!-

NRC INSPECTICN REPORTS DATING 8ACK 701979 FOUND PROBLEMS AT THE PRCPER CUALITY ASSURANCE CONTROLS WERE NOT EMPLOYED IN TECHNICAL ScuTH TEXAS PLANT SIMILAR TO THOSE IDENT!FIED IN THE QUADREX AND PROCUREMENT COMMUNICATIONS WITH SERV!CE-TYPE CONTRACTORS. NOR REPORT. HowEVER, THE AGENCY'S AUDITS D!D NOT SURFACE THE NUMBER WERE DOCUMENT CONTROLS ADECUATE TO ASSURE THAT THOSE INVOLVED IN l OF PROBLEMS SUGGESTED BY THE QUADREX REPORT. THouGH WE WERE ESIGN HAD READY ACCESS TO THE MOST RECENT INFORMATICN AVAILABLE.
AWARE OF CA PROBLEMS AT SOUTH TEXAS AND HAD CITED THE LICENSEE '1 FOR A 8REAKD0mM IN THEIR CA PROGRAM IN APRIL 1980. THE MAGNITUDE BECAUSE OF THE INADECUACY OF CA CONTROLS CVER DESIGN VERIFICA-CF POTENTIAL PRCSLENS WAS NOT FULLY APPRECIRTED UNTIL WE FIRST REVIEhED THE REPORT IN AUGUST OF 1981. CONTRACTORS. THE ACCEPTABILITY OF THE DESIGNS BASED CN THEIR ANALYSES IS NOW IN CUESTION.
