Dear Sir:

Inservice Testinn of Component Cooling Water Safety Valve Your letter of October 28, 1985 requested documentation regarding repair actions accomplished on safety valve PSV-3323B following the failure of this valve during Inservice Testing (IST). Please note that it was PSV-3323A which was tested in 1985 and which failed the test.

The failure was due to the valve setpoint drifting low.

PSV-3323A has a esq. ired set-point of 150 i3 psis. The as-found setpoint during the IST was 145 psig.

Corrective action was taken to adjust the valve setpoint and the valve was then retested satisfactorily.

Since the only problem with PSV-3323A was a slight drift in the setpoint, there was no reason to suspect a common mode failure. However, in recognition of the potential for these valves (PSV-3323A and B) to experience unacceptable drift in less than the normal test interval of 5 years, the IST for PSV-3323B was accelerated to 1986 instead of 1987 in accordance with the provisions of Section XI of the ASME Boiler and Pressure Vessel Code.

Further consideration regarding the responsibility for IST has led to the decision that responsibility for the IST program for pumps and valves (PGE-1022) will be with the Trojan Plant staff. This is a change to the position in our letter dated September 26, 1985.
