Enclosed is License No. SNM-1962 which authorizes you to possess in the United States the plutonium-238 contained in your pacemaker. Also enclosed is a medical alert card that the license requires you to carry while you are in the United States.
Since you reside near the United States border and may visit this country frequently, you may keep the medical alert card for use on future visits. It will not be necessary to return the card after each visit as was stated in Item V of your original application.
