Operating License No. DPR-73 Docket No. 50-320 Defueling Canisters As originally discussed with you and members of your staff during the week of November 18, 1985, problems have been encountered relative to the verification of the proper loading of B C poison into poison tubes by 4
Nuclear Energy Services (NES) for use in filter and knockout canisters at TMI-2. GPU Nuclear is currently inquiring into this matter and will provide the results of that inquiry in a future letter.
In the interim, GPU Nuclear has developed a program to perform an independent verification of poison tube loading for those poison tubes loaded by NES. This program, which was formally instituted on December 4, 1985, is discussed in the Attachment to this letter. Defueling canisters, which are excluded from this program, are specifically identified in the Attachment.

presence of poison in these tubes is being performed by the resident Supplier Quality Representative (SQR) in accordance with the approved Quality Surveillanco Plan. GPU Nuclear QA, as part of the on-site receipt -inspection, will require documentation of this -independent verification. GPU Nuclear QA also will require documentation of the independent verification of the presence of catalyst for all canisters.
Docunentation supporting the program described in this letter will be retained on-site for your review. Therefore, based on discussions with your staff, GPU Nuclear understands that we are no longer required to notify your staff of future acceptance of canisters.
In accordance with GPU Nuclear letter 4410-85-L-0228 dated November 12, 1985, GPU Nuclear will notify your staff of any planned reduction in the augmented Quality Assurance' review program for fES supplied canisters.

Independent Verification Program for Filter and Knockout Canisters -
Due to the uncertainty in the percentage of independent verifications performed on poison (8 C) tubes loaded at NES, GPU Nuclear plans to perform 4
radiographic examinations on all canisters which contain poison tuoes loaded at NES.. Currently, this program will encompass radiographing twenty-one (21) filter canisters from B&W, fourteen (14) filter canisters from NES, and twenty-seven (27) knockout canisters from NES. Within the limits implicit in the radiographic examinations, this program is intended to provide reasonable assurance of the presence of poison pellets in the canisters. By design, fuel canisters do not contain poison pellets and, therefore, are excluded from this program.
The specifics of this program are as follow:
o A " full-length" RT will be performed on all knockout canisters which
'contain poison tubes loaded at NES.
o A " full-length" RT also will be performed on four (4) B&W filter
-canisters which contain poison tubes loaded at NES and are currently awaiting shipment to TMI-2. GPU Nuclear decided to perform a
" full-length" RT on these canisters in order to expedite shipment in support of the TMI-2 defueling schedule pending our evaluation of the RT program.
o All remaining filter canisters which contain poison tubes loaded at NES will have a RT performed only on the top section of the poison tube in order to verify the presence of pellets. GPU Nuclear believes that the extensive RT program for the knockout canisters is not required for the remaining filter canisters since the filter canister criticality analyses demonstrate that poison material is not essential-to maintain subcriticality in the filter canisters.
Based on successful completion of this program, GPU Nuclear believes that the requirements for independent verification of poison tube loading for these poison tubes, as required by your letter NRC/TMI-85-081, dated November 8, 1985, will have been satisfied.
In addition to the poison tubes loaded at NES, other poison tuoes are being loaded at Joseph Oat Corporation. Independent verification of the presence of poison in these tubes is being performed by the resident Supplier Quality
. Representative (SQR) in accordance with the approved Quality Surveillance Plan. Therefore, ~ radiographic examination of these poison tubes is not required.
Canisters Accepted to Date and Basis for Acceptance To date, the following canisters have been accepted by GPU Nuclear and released for unrestricted use:
o Four filter canisters from NES (F-401 through F-404). NRC letter dated November 8, L1985, granted approval for use of these canisters based, in part, on the NRC's determination that the licensee had provided reasonable assurance that catalyst recombiners and poison pellets had been installed in these canisters.

o GPU Nuclear letter 4410-85-L-0229. dated Novemoer 12, 1985, advised the 2C that. independent verification.of the catalyst loading into the canisters had been confirmed as required by your letter of. November'8, 1985. This information w'as restated in GPU Nuclear letter 4410-35-L-0228 date'd November-12, 1985, which additionally advised that these canisters had been accepted by EG&G, Idaho, Inc. and GPU Nuclear Quality Assurance.
