APPENDIX A NOTICE OF VIOLATION Madison Paper Company Docket No. 99990001 Madison, Maine 04950 General License As a result of the inspection conducted on August 15, 1985, and in accordance with the NRC Enforcemer.t Policy (10 CFR 2, Appendix C), the followirg violations were identified:

10 CFR 31.5(c)(2) require. that the proper operation of the on-off mechanism and indicator be checked at no longer than six months intervals.

Contrary to the above, as of August 15, 1985, two gauges containing 1 curie and 0.3 curies of Krypton-85 did not have the on-off mechanism and indicator tested from December 1984 to August 1, 1985, for the one gauge and since December 1984 for the other, intervals of more than six months.

This is a Severity Level IV violation (Supplement VI).

Pursuant to the provisions of 10 CFR 2.201, Madison Paper Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) the corrective steps which have been.taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where gcod cause is showr, consideration will be given to extending this response time.
