Georgia Power Company (GPC) hereby submits the following information in response to Inspection Report 86-02, which concerns the inspection conducted by Mr. B. R. Crowley of your staff on January 6-10, 1986 One apparent violation was identified and was applicable to Hatch Unit 1 only.
VIOLATION 10 CFR 50, Appendix B, Criterion Y, as implemented by section 5 of the Hatch Nuclear Plant Quality Assurance Manual, requires that activities affecting quality shall be prescribed by documented instruction, procedures, and drawings and shall be accomplished in accordance with these instructions, procedures and drawings.
Paragraph 2.5 of Nutech procedure 9-4, " Process Control", the applicable procedure for control of work activities (Induction Heat Stress Improvement (IHSI)), requires that subcontractors sign the traveler indicating that their described operation is completed.
Contrary to the above, on January 8,1986, activities affecting quality were not being performed in accordance with documented procedures in that for IHSI of welds 1831 -1 RC-12AR-G-1, 1831 -1 RC-12AR-F-1, 1831-1RC-22AM-3BC-1, 1 B31 -1 RC-12AR-K-1, and 1831-1 RC-28A-16, Nutech signed off as the performing craft on traveler steps performed by their
. subcontractor.
This is a Severity Level V violation (Supplement I).

RESPONSE
Admission or denial of alleged violation: The violation occurred.
Reason for violation:
The individuals involved in performance of the work activities did not understand the proper methods for signing off steps on a procedure data package.
The subcontractor who performed the work told the verifier that the work was complete.
The verifier incorrectly signed the work performance steps for the subcontractor as well as signing the verification.
Corrective steps which have been taken and the results achieved:
The contractor-QA program discovered this i tem and initiated prompt corrective action on January 9, 1986 A field change was initiated correcting the signatures to reflect
-the signatures of the subcontractors who actually performed the work.
A training session was held for contractor shift supervisors.and subcontractor foremen on this subject.
Hatch management has issued a
Management Memo (MM-MGR-002-0186) dated January 15, 1986, to caution plant personnel on the proper protocol for signing documents affecting quality.
Al so, a Departmental Directive (DGit 86-06) dated February 10, 1986, was given to plant ' personnel to instruct them on signing documents.
NRC I&E Information Notice 85-97,. " Jail Term For Contractor Employee Who Intentionally Falsified Welding Inspection Records", was disseminated to plant personnel to demonstrate the potential consequences of improperly signing a document affecting quality.
Corrective steps which will be taken to avoid further violations:
The above actions are sufficient to prevent recurrence.
Date when full compliance was achieved:
The plant achieved full compliance When the appropriate signatures were obtained on the five subject weld packages on January 10, 1986.
