. s ENCLOSURE 1 NOTICE OF VIOLATION Mississippi Power & Light Docket No. 50-416 Grand Gulf Nuclear Station License No. NPF-29 The following violations were' identified during an inspection conducted on February 25 - March 17,1986. The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

1. Technical Specification 6.8.1 requires written procedures be established and implemented covering surveillance and test activities of safety equipment recommended in Appendix A of Regulatory Guide 1.33, Revision 2, _ February 1978.

Contrary to the above:

a. On February 20, 1986 Surveillance Procedure 06-EL-1E12-M-0002, Contain-ment Spray Time Delay Relay Calibration And Functional Test, was not properly implemented in that electricians erroneously connected and verified test switch electrical leads that resulted in initiation of Division 2 Emergency Core Cooling System (ECCS) equipment.

b. On October 27, 1985, Administrative Procedure 01-S-07-1, Control of Work on Plant Equipment and Facilities, was not properly implemented in that restoration of leads on a Troubleshooting Log Sheet was not properly documented and verified.

This is a Severity Level IV (Supplement I).

2. 10 CFR 50, Appendix B, Criterion XVI requires that measures be established to assure that conditions adverse to quality are promptly identified and corrected.

Contrary to the above, corrective action was not accomplished until March 1986, for the certification of a non-seismic qualified relay in the control room outside air intake damper system that was identified on April 19, 1982.

This is a Severity Level V (Supplement I).

Pursuant to 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
