NRC-10 87-335 Modification No.12 Page 2 of 3 The purpose of this modification is to: (1) extend the period of performance, effective 5/17/97 through 5/16/98; (2) revise the Statement of Work to incorporate an option year, specify supply requirements, identify Government furnished property, and revise the reporting requirements, as attached; and (3) provide funding in the amount of $3,946.00, which increases the cooperative agreement ceiling and total obligated amounts from $47,646.48 to $51,592.48. Accordingly, the following changes are hereby made to the " Notice of Cooperative Agreement":
- 1. Item No.15, Funding Sources. is deleted in its entirety and replaced with the following:
