In accordance with 10CFR55.23 and 10CFR55.33, Southern Nuclese Operating Company (SNOC) is submitting a revised Certification of Medical Examination form (NRC Form 396) for License NSOP 20857/ Docket #55 21373. A recent eye examination performed on April 16, 1997, indicates that the individual now requires corrective lenses when performing licensed duties.
