Dear Dr. Murley:
This special report, required by Technical Specification 3.5.1.f and 4.7.3.d, documents nine (9) High Pressure Coolant Injection (HPCI) System injections and three (3) Reactor Core Isolation Cooling (RCIC) systan actuations associated with the scram of Unit 2 on December 2,1985. The loss of a transformer caused a manntary power loss to the feedwater control circuitry and 'A' reactor recirculation pump circuitry. This resulted in a feed flow-steam flow mismatch. The Unit 2 reactor scranmed on low vessel water level when attenpts to restore feedwater level control were unsuccessful. The HPCI and RCIC systens were manually started to control reactor vessel level and pressure. A total of nine (9) HPCI and three (3) RCIC injections were made. The scram was previously reported under Licensee Event Report (LER) 85-034-00.
As of December 2,1985, the total number of HPCI actuations for Unit 2 is twenty-three (23); RCIC injections total fifty-tm (52). A conservative analysis indicates the current usage factor for all affected nozzles is less than 0.70.
