d APPENDIX A NOTICE OF VIOLATION Kansas Gas and Electric Company Docket No.. 50-482/85-30 Wolf Creek Generating Station License No. . NPF-42 During'an NRC inspection conducted during the period of October 21-25, 1985, two violations of NRC requirements were identified. The violations involved violation of Technical Specification and failure to follow surveillance test

procedure. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below

A. Violation of Technical Specification - Fire Suppression System Surveillance Technical Specification (TS) 4.7.10.2.a requires that the water spray fire suppression system for ESF transformers XNB01 and XNB02 be demonstrated operable at least once per 31 days by verifying that each valve in the flow path is in its correct position.

Contrary'to the above, Surveillance Test Procedure STS FP-005, Revision 0,

" Monthly Sprays / Sprinkler Valve Position Verification," does not require an inspection of these two ESF transformer water spray systems. Thus, the required TS monthly surveillances have not been performed.

This is a Severity Level IV violation (Supplemer.t-I.D) (50-482/8541-01).

i B. Failure to Follow Surveillance Test Procedure for Electric Motor Driven

] 10 CFR Part 50, Appendix B, Criterion V, states, in part, " Activities affecting quality shall be prescribed by documented instructions,

' procedures, or drawings . . . and shall be accomplished in accordance with these instructions, procedures, or drawings . . . " .

STS FP-602, Revision 3, " Electric-Motor Driven Fire Pump'1FP01PA-Monthly Operation," requires in Sections 5.2 and 5.3 that the fire pump be started 4

using the switch on the local panel during the monthly tests for January, March, May, July, September, and November. _

During the alternate months the procedure requires that the pump be started from the control room panel.

Contrary to the above, the electric motor-driven fire pump was ' tested by

, starting it from the control room panel'for May 1985 and from the local j; panel for June 1985.

This is a' Severity Level _ V violation (supplement 1.E) (50-842/8541-02).

2 Pursuant to the provisions of 10 CFR 2,201, Kansas Gas and Electric Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
