SUBJECT:

SUMMARY

OF THE OPERATING REACTORS EVENTS MEETING ON NOVEMBER 18, 1985 - MEETING 85-23 On November 18, 1985, an Operating Reactor Events meeting (85-23) was held to brief the Office Director, the Division Directors and their representatives on events which occurred since our last meeting on November 4,1985. The list of attendees is included as Enclosure 1.

The events discussed and the significant elements of these events are presented in Enclosure 2.

In addition, the assignment of follow-up review responsibility was discussed. The assignments made during this meeting and the status of previous assignments are presented in Enclosure 3.

Completion dates have been assigned for items in Enclosure 1. Each assignee should review Enclosure 3 with regard to their respective responsibilities.

Note that several assignees are approaching the due date. Please be responsive and advise ORAB (D. Tarnoff, x29526) if the target completion date cannot be met.

l POSSIBLE GENERIC CONCERN REGARDING MAINTENANCE PROCEDURES FOR WOODWARD GOVERNOR.

OVERSPEED TRIP FUNCTION MAY NOT PROTECT AGAINST EXCESSIVE OVERSPEED DURING FAST ENGINE STARTUP IF ELECTRIC GOVERN 0R IS INOPERABLE.

CIRCUMSTANCES:

ENGINE OPERABILITY POST-MAINTENANCE TEST DURING PLANT OUTAGE FOLLOWING SCHEDULED PREVENTIVE MAINTENANCE OF GOVERN 0R UNEXCITED START - NO ELECTRIC GOVERNOR FUNCTION PER 2

PROCEDURE, FUEL RACK FULL OPEN, TRIPPED ON OVERSPEED AND MANUALLY, TOTAL RUN TIME 45 SEC, SEVERE VIBRATIONS.

WOODWARD MANUALS IN USE AT GGNS DID NOT SPECIFY VENTING /

BLEEDING AIR FROM GOVERNOR DURING P/M.

VENTING / BLEEDING IS SPECIFIED IN LATEST WOODWARD MANUALS.

MECHANICAL GOVERNOR WAS INCORRECTLY SET TO 470 RPM DURING TEST RATHER THAN TO 350 RPM AS CALLED FOR BY NORMAL POST-PM PROCEDURE.

. 2 FOLLOWUP PEAK ENGINE SPEED UNDER INVESTIGATION ENGINE DISASSEMBLY AND INSPECTION BEING PERFORMED, NUMEROUS PARTS IN DAMAGED OR DISTRESSED CONDITION: PARTICULARLY CONROD BEARINGS, MAIN BEARINGS, LINK PIN BUSHINGS, AND ENGINE BASE, WILL REPLACE ALL CONROD AND MAIN BEARINGS, ONE CONROD, AND OTHER PARTS AS NECESSARY/ PRUDENT, SIGNIFICANCE OF ENGINE BASE CRACKS ARE UNDER EVALUATION SENT GOVERNOR TO MANUFACTURER (WOODWARD) FOR TESTS IN AS-FOUND CONDITION (INCLUDING INSUFFICIENT OIL),

PROBLEM:

FAILURE OF MAIN BEARINGS IN EDG NO. 13 (DIV.2) CAUSED SEVERE DAMAGE SAFETY SIGNIFICANCE:

LOSS OF AN EDG WILL CAUSE LOSS OF ONE DIVISION OF ON-SITE POWER SUPPLY CASTS DOUBT ON RELIABILITY OF ON-SITE POWER SUPPLY CIRCUMSTANCES:

EDG FAILURE OCCURRED 2 1/2 HRS. INTO A 24-HOUR LOAD TEST OPERATOR SECURED EDG UNIT UPON HEARING " UNUSUAL" NOISES BACKGROUND:

UNIT HAD A NUMBER OF INADVERTENT STARTS WITHOUT PRELUBE SINCE LAST INSPECTION IN FEBRUARY 1985 3 0F 4 EDG's WERE INADVERTENTLY STARTED WITHOUT PRELUBE ON 11/10/85 EDG NOS. 11 AND 12 (DIV.1) HAD SIMILAR FAILURES IN JANUARY 1985 HOWEVER, LOWER BEARING HAS FAILED IN THIS EVENT, NO LOWER BEARING FAILURE OBSERVED PREVIOUSLY FOLLOWUP:

REGION III, NRR, FAA, AND DECO ARE SENDING SAME PERSONNEL WHO INVESTIGATED PREVIOUS FAILURES N0. 3 UPPER MAIN BEARING FAILED NO. 4 LOWER MAIN BEARING FAILED NO. 13 UPPER BEARING (THRUST) IS " DISTRESSED" DISASSEMBLY IS CONTINUING

ST, LUCIE UNIT 1 - INTERNALS LIFTING RIG FAILURE NOVEMBER 6, 1985 (S. LONG, IE)

PROBLEM:

WHILE REMOVING THE UPPER INTERNALS GUIDE STRUCTURE 1 0F 3 BOLTS CONNECTING THE STRUCTURE TO THE LIFTING RIG FAILED WITH THE STRUCTURE STILL OVER THE CORE SIGNIFICANCE:

THE 50 TON LOAD COULD FALL BACK INTO REACTOR VESSEL, POSSIBLY CONTACTING AND DAMAGING FUEL, RESULTING IN RELEASE OF RADI0 ACTIVE GASES TO THE CONTAINMENT CIRCUMSTANCES:

WHILE LIFTING INTERNALS, LIFT WAS STOPPED WHEN RIG WAS SEEN TO BE SLIGHTLY TILTED DYNAMIC FORCE FROM STOPPING LIFT CAUSED 1 0F 3 BOLTS ATTACHING RIG TO INTERNALS TO FAIL.

RIG CANTED -se N6" UP AND STRUCTURE s' N6" DOWN ATTEMPT WAS MADE TO LOWER INTERNALS TO INSTALLED POSITION, BUT LOAD CELLS INDICATED BINDING LICENSEE DECLARED AN UNUSUAL EVENT, ENHANCED CONTAIN-MENT INTEGRITY AND IMPLEMENTED PROCEDURES TO MONITOR LOAD STABILITY SECONDARY LIFTING RIG WAS DESIGNED, TESTED AND INSTALLED ASSEMBLY WAS JACKED LEVEL AND THEN MOVED TO ITS NORMAL POSITION IN REFUELING P0OL ON 11/9 FOLLOWUP:

UNDERWATER CAMERA SURVEY INDICATED THAT FAILED BOLT WAS ONLY INSERTED ABOUT THREE TURNS, AND STRIPPED DURING LIFT CE PROCEDURES FOR RIG ATTACHMENT INCLUDE VERIFICATION OF THREAD ENGAGEMENT; THIS WAS OMITTED FROM LICENSEE'S PROCEDURES KNOWN DAMAGE:

LIFTING RIG IS BENT.

PROBLEM:

CONTAMINATION OF INDIVIDUALS DURING EMERGENCY DRILL SAFETY SIGNIFICANCE:

PERSONNEL CONTAMINATION DISCUSSION:

TVA WAS SUPPORTING A STATE OF ALABAMA MEDICAL DRILL DURING AN EXERCISE OF THE BROWNS FERRY EMERGENCY PLAN.

SCENARIO CALLED FOR A MONITORING TEAM TO RESPOND TO A CONTAMINATED CASUALTY OFFSITE.

CONTAMINATION WAS SUPPOSED TO BE CONTAINED SOURCES.

A STATE OF ALABAMA EMPLOYEE WHO WAS ACTING AS AN EXERCISE CONTROLLER DRIPPED ABOUT ONE MILLICURIE OF LIQUID TECHNETIUM 99M ON THE BANDAGE OF A TVA EMPLOYEE WHO WAS PLAYING THE ROLE OF THE CASUALTY, ANOTHER MILLICURIE OF TE-99M WAS DRIPPED ON TO A LAB C0AT.

TE-99M IS A LOW ENERGY GAMMA EMITTER WITH A HALF-LIFE OF SIX HOURS THE SIMULATED CASUALTY ALONG WITH FOUR OTHER TVA EMPLOYEES ON THE RESPONSE TEAM BECAME CONTAMINATED IN THE COURSE OF HANDLING THE CASUALTY, THE INCIDENT TOOK PLACE ABOUT ONE AND A HALF MILES OFFSITE.

THE CONTAMINATION OCCURRED ABOUT 12:30 PM.

INDIVIDUALS RETURNED TO PLANT FOR DECONTAMINATION.

A NOTICE OF UNUSUAL EVENT WAS DECLARED AT 5:28 PM.

ONE INDIVIDUAL HAD CONTAMINATION TO 30,000 CPM ON HIS FACE AND 500,000 CPM ON ONE HAND, CONTAMINATION LEVELS RANGED FROM 800 TO 40,000 CPM ON THE OTHER FOUR INDIVIDUALS, THE ALABAMA STATE EMPLOYEE RECEIVED CONTAMINATION ON HIS SHOES.

A 200 SQUARE FOOT AREA 0F GROUND WAS ALSO CONTAMINATED.

SOME CONTAMINATION WAS INGESTED, INITIAL LICENSEE REPORT INDICATED ONE INDIVIDUAL RECEIVED ABOUT A 4% BODY BURDEN, TVA PERFORMED WB COUNTS AND IS COLLECTING URINE AND FECAL SAMPLES.

THE 200 SOUARE FOOT AREA WAS POSTED AS A CONTAMINATED AREA, FOLLOWUP:

TVA IS PLANNING TO MEET WITH THE STATE AND IS PREPARING AN INTERNAL REPORT.

NOT CONSIDERED A REPORTABLE EVENT, STATE OF ALABAMA IS PREPARING A REPORT.

ALABAMA IS AN AGREEMENT STATE.

A PREVIOUSLY SCHEDULED REVIEW 0F THEIR PROGRAM BY REGION II IS TO TAKE PLACE THIS WEEK, NO ADDITIONAL ACTIONS CONTEMPLATED AT THIS TIME, i
