We have reviewed your corrective actions, which appear to be adequate, and have no further questions at this time. These corrective actions will be examined during a future inspection.

Dear Mr. Phillips:

At the time the violation occurred, the records pertaining to the rate meter calibrations were being moved to our office in Arkansas. This made it difficult to monitor the expiration date of the rate meters being used by our out of state radiographers. When I was notified that the radiographers rate meter was out of calibration, I immediately over-night Fed-Exed a properly calibrated rate meter to the radiographer.

The radiographer was also reprimanded for not more closely monitoring his rate meter calibration due date since our survey reports contain a space for recording that date.

As of July, 1996, I vacated my New Mexico license and all my records have been moved to our Odessa, Texas office. All the records are maintained in one office, and are overseen by one  ;

person; the Radiation Safety Officer for my Texas license. ]

Full compliance to 10 CFR 2.201 was achieved in July, 1996 by centrally locating all of the company records. This should insure that this violation never re-occurs.
