September 24, 1985 EN 85-67 0FFICE OF INSPECTION AND ENFORCEMENT NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Licensee: Comonwealth Edison Company LaSalle County Nuclear Power Station, Units 1 and 2 Docket Nos. 50-373 and 50-374

PROPOSED IMPOSITION OF CIVIL PENALTY - $125,000 This is to inform the Comission that a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of One Hundred and Twenty-five Thousand Dollars ($125,000) will be issued on or about September 27, 1985 to Comonwealth Edison Company. This action is based on three examples where Comonwealth Edison Company failed to ensure that modifications performed on safety-related systems were adequately controlled so that the operability of the systems was not jeopardized. The first example involves inadequate controls in the design, inspection and testing areas that resulted in the piping to two reactor vessel water level actuation switches in Division I of the Unit 2 Emergency Core Cooling System (ECCS) being installed backwards. The Division I ECCS pumps would not have automatically initiated on a low-low-low reactor vessel water level trip signal and the other two ECCS Divisions were also unavailable.

Unit 2 was in cold shutdown at the time. The other two examples involve similar design and testing failures in the modification program.

It should be noted that the licensee has not been specifically infomed of the

! enforcement action. The Regional Administrator has been authorized by the l

Director, Office of Inspection and Enforcement, to sign this action. The schedule of issuance and notification is:

l A news release has been prepared and will be issued about the time the I

licensee receives the Notice. The State of Illinois will be notified.

The licensee has thirty days from the date of the Notice in which to respond.

Following NRC evaluation of the response, the civil penalty may be remitted, mitigated, or imposed by Order.
