This is in reference to your request in a letter to renew License No. SNM-356.

In order to continue our review, we need the following additional information:

1. Regarding your area radiation and contamination surveys, please describe

~

the criteria which will be used to determine the frequency of surveys required by individual investigators. Also describe the contamination limits in units of dpm /100 cm above 2 which decontamination of the area will be required.

2. Please describe the criteria you use to determine which personnel are required to wear dosimeters and what type of dosimeters are required.
3. -Describe your-procedures for providing instruction as required in 10 CFR 19.12 for laboratory workers, including post-doctoral research staff, and for ancillary personnel such as security and custodial staff. Confirm that all individuals who work in or frequent restricted areas will receive initial'and annual refresher training and describe the method you will use to document and audit training.
4. If you propose to calibrate your own radiation survey and monitoring instruments, you should submit a detailed description of your planned calibration procedures. The description of calibration procedures should

- ' include, as a minimum:

a. The manufacturer and model number of the sources (s) to be used.
b. The nuclide and quantity of radioactive material contained in the source.
c. The accuracy of the source (s). Traceability of the source to a primary standard should be provided.
d. 'The step-by-step procedures, including associated radiation safety procedures. 'These procedures should include a two point calibration of each scale of each instrument with the points' separated by at least 50% of the scale.
