SUMMARY
Review of $4EC drawing 12241-RE-28A-4, details E-1, F,
and G which I
specify the methad and materials for 5 kV power cable tenninations to motors and other miscellaneous equipment with pigtails, indicated that the specified nunber of layers of insulation tape did not agree with the i
4 manufacturer's (Kerite Company) specified requirements.

IMMEDIATE ACTION TAKEN On July 18, 1985, E&DCR 2P-4757 was issued to modify all suspect 5 kV terminations made under details G, E-1, and F on drawing 12241-RE28A-4.

Department, notified Mr. G. Walton, NRC Region I office, by telephone on 1
August 14, 1985, that DLC considerei this matter to be potentially reportable under 10CFR50.55(e).

DESCRIPTION OF THE DEFICIENCY i
j An incorrect nunber of layers of insulating tape was specified on SWEC drawing 12241-RE23A-4 for 5 kV tenninations under Details E-1, F, and G.
Forty-six (46) tenninations had been made due to the erroneous drawing specifications, i
j 4.
ANALYSIS OF SAFETY IMPLICATION The deficient taping procedures were used at the electrical connection to various safety-related 4.16 kV motors.
The total thickness of insulating tape specified on drawing 12241-RE-23A-4 waald be approximately 120 mil j
as compared to 500 mil specifteti by the manufacturer (Kerite Company).

Most of the deficient tenninations were located -in, or were scheduled to i
be located in a harsh environment. The significant reluction in insulat-ing tape thickness below the manufacturer's requireaents is considered to l
be sufficient basis for assuming the tennination could f ail prenaturely.
1 Failure of the tennination could result in loss of operation of a con-j nected 4.15 kV safety-related motor.

CORRECTIVE ACTION TO REMEDY DEFICIENCY I
The fif teen (15) 5 kV cable tenninations turned over to the Start-up Group (SVG) have. been reorked in accordance 'with E&DCR 2P-4757 to l
achieve the proper nu:noer of layers of insulating tape, thus bringing the L
terminations in agreement with the manufacturer's requirenents.
The remaining 31 tenninations will be reworked prior to turnover to SVG.

Drawing 12241-RE-28A-4 nas been revise.1 such that detalls E-1, F, and G reflect tne manuf acturer's 5 kV tennination i eilistranents.
In addition, all other details on suspect tennination drawinjs have been reviewed for consistency with manufacturer's requirements.
6.
ADDITIONAL REPORTS This is the final report regarding the insuf ficient thickness of insu-lating tape on 5 kV power cable terminations.
