Enclosed is the preliminary (i.e. unsigned) Personnel Qualificaten Statement (NRC Form 398) for the Reactor Operator (RO) license candidate identified below. This candidate faBed the Category B section of the license emm given at Limenck during the week of January 21,1997 - January 24, l

1997. Detals of the waiver request for the portions of the emm on which the candidata passed are contained on the NRC Form 398. A current Certificate of Medical History (NRC Fonn 396), dated 11/27/96 is on file with the NRC.

A tentative re-emmination date has been set for May 1,1997.
