ENCLOSURE 1 NOTICE OF VIOLATION Mississippi Power and Light Company Docket No. 50-416 Grand Gulf Nuclear Station License No. NPF-29 The following violation was identified during an inspection conducted on December 21, 1985 - January 17, -1986.

The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

' Techn'i cal Specification 6.8.1 requires written procedures covering activities recommended in Appendix '"A" of Regulatory Guide 1.33, Revision 2, February 1978.

Regulatory Guide 1.33, Appendix "A" recommends Administra-tive Procedures for Authorities and Responsibilities for Safe Operation and Shutdown and General Plant Operating Procedures for Cold Shutdown to Hot Shutdown.

Integrated Operating Instruction 03-1-01-1, cold shutdown to generator carrying minimum load, requires, with shutdown cooling in service, that reactor coolant temperature be limited to 150 F or less and that reactor vessel pressure, thermal power and reactor coolant temperature -be recorded on data sheet III.

It also requires in a caution statement that the

- feedwater pump turbine speed be increased as necessary to. maintain the feedwater pump discharge pressure 100 psig above reactor pressure.

-Administrative Procedure 01-S-06-2, Conduct of Operations, Step 6.3.6 requires that one licensed operator be dedicated to monitoring important parameters such as reactor water. level during a startup. Licensed operators are trained' that proper monitoring of a parameter such as water level requires monitoring of all available instrumentation for that parameter.

Contary to the above:

a.

On December 22, 1985, the B residual heat removal system was placed in the shutdown cooling mode with the recirculation loop suction tempera-ture greater than 150 F and data sheet III was not used.

b.

On January 1, 1986 during a reactor startup, feedwater discharge pressure was not maintained 100 psig above reactor pressure.

This contributed to a low water level reactor scram.

On January 1,1986 during a reactor startup, the licensed operator dedicated to monitoring reactor water. level monitored only the level recorder which was stuck at normal operating level and not the other three ' water level meters that indicated a decreasing water level.

Without the operator taking action, this resulted in. a low water level reactor scram.

Pursuant to 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice, a wri.tten statement or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for

-the violations if admitted; (3) the corrective steps which have been taken and

. the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from the public disclosure as required by 10 CFR 73.21.
