0 Office of Nuclear Reactor Regulation Items of Interest Week Ending March 28. 1997 MNTA Meetina Dr. George Usova of the Operator Licensing Branch (HOLB) participated in a Midwest Nuclear Training Association (MNTA) meeting on March 25, 1997.

Dr. Usova presented a session on how to write test questions for NRC operator r licensing examinations. The presentation was in support of the agency efforts to increase licensee participation in the operator licensing examination process.

Maine Yankee By letter dated March 7. 1997 the licensee submitted Revision 0 of its Restart Readiness Plan (RRP). The body of the plan provides a broad overview of the licensee's process to identify and resolve issues prior to restart.

The licensee has established three committees to implement the process.

The Hardware Subcommittee will review equipment, system, and material condition issues against established criteria and classify them as either restart, short-term (within 6 months after restart) or long-term (Business Plan) issues. The Non-Hardware Subcommittee will review program process, and ,

human performance issues against the same criteria and classify them appropriately. These two subcommittees are made up of line managers. They will also be responsible to review and approve all corrective actions in their respective areas of responsibility.

The Restart Steering Committee (RSC) is made of senior managers and executives and is responsible to support the Vice President of Operations in the overall direction and oversight of the restart program. They will evaluate, screen, and close out major issues. They will also review and approve the activities of the subcommittees.

The Appendices of the RRP discuss " Action Plans for Significant Restart l Issues." The issues discussed in the appendices in Revision 0 are logic circuit testing (GL 96-01), cable separation, offsite power. steam generator inspections, failed fuel corrective action program, surveillance testing, and environmental qualification. The format of the RRP is designed to allow addition of new Appendices for emergent significant restart issues as the outage proceeds.

The staff will meet with the licensee in the afternoon of April 3.1997, to discuss the RRP. That meeting will be open for public observation. On the evening of April 3.1997, the staff will meet with interested members of the public to receive public comments on the RRP. After these meetings, the staff will develop its restart plan pursuant to Manual Chapter 0350.

On Friday. March 21. 1997, the De)uty Director of DRPE visited the site and discussed a variety of issues wit 1 licensee staff. In the fire protection area, the licensee noted that reviews it has conducted since the Spring of  ;

1996 have indicated that approximately 90% of 2600 fire barrier penetration seals will need to be replaced, repaired, or reanalyzed. Defects with the silicone foam used to form the barrier include inadequate thickness, bad cell structure, gaps and voids, foreign material, and poor work control when the sealant was being put in place. The licensee has developed a plan to address this issue after cable separation concerns are resolved. The licensee estimated that at least 1600 fire barrier penetration seals will need to be physically changed.

Status of Review of Licensee Responses to 50.54(f) Recuest on Desian Bases Phases 1 and 2 of the process for reviewing licensee responses to the 10 CFR 50.54(f) request for information on design bases are complete.

Regions II and III presented the results of their Phase 3 reviews at their Plant Performance Review meetings the week of March 10, 1997. Region I 3 resented the results of their Phase 3 review at their Plant Performance Review meeting the week of March 24. 1997. Region IV will present the results of their review at a 10 CFR 50.54(f) Assessment meeting the week of March 31, 1997 Results of the Phase 3 reviews for all Regions will be discussed at the Senior Management Meeting screening meetings scheduled for April 1997.

NRC Insoector and Licensee Reoresentative Exoosed to HF Gas

! Between 9:30-10:00 am on Mar'ch 26, 1997, while conducting.a material control

, and accountability inspection in the cylinder yard at the ABB Combustion Engineering fuel cycle facility in Hematite, Missouri, an NRC inspector and a 4

licensee representative were exposed to hydrofluoric acid (HF) gas. The HF gas was released due to a-leak in the HF dry scrubber system next to the 4

cylinder yard. The licensee representative complained of nausea and was sent home. He returned to work the next day. The NRC inspector experienced a

! . burning sensation in the nose and throat at the time of the event. Medical t attention was sought as a precautionary measure. Examination results

identified a mild nasal irritation. The NRC Health and Safety Manager has i been notified.

j- Safety Insoection of VSC-24 Dry Soent Fuel Storaae System On March 17-21, 1997, Spent Fuel Project Office (SFPO) staff conducted a i safety inspection of the VSC-24 spent fuel storage system at Sierra Nuclear i Corporation (SNC) and two of its subcontractors in the Scotts Valley, t California area. The inspection was conducted to follow up on incidents of cask seal weld failures at Palisades and Arkansas Nuclear One and to verify

implementation of corrective actions for findings identified during two

, previous inspections performed at SNC in June and August 1996.

l- The-inspection team identified significant findings regarding: (1) SNC's i failure to conduct a rcot cause analysis of the weld 3roblem: (2) failure to i examine the potential generic aspects of the weld pro)1em: and (3) failure to correct the " time to drain down" requirements to account for reali3 tic pool

! water temperatures. The inspection also identified other findings of lesser significance. '

An exit meeting was held on March 21, 1997, to present the preliminary inspection findings. Representatives from three utilities using the VSC-24 dry s)ent fuel storage system also participated in the exit meeting by teleplone, SFP0 is currently reviewing the need for follow-up action.

American Society for Nondestructive Testina

On March 21, 1997, the Chief Medical, Academic and Commercial Use Safety 1 Branch, Division of Industrial and Medical Nuclear Safety, participated in The Spring Conference and Annual Research Symposium held in Houston, Texas, by The American Society for Nondestructive Testing (ASNT), He provided presentations entitled, "Recent Changes in 10 CFR Part 34." and "How to Comply with ANSI N-432 Requirements," both dealing with industrial radiography. He also took part in organized discussions dealing with the use of alarming rate

< meters for industrial radiography personnel and with the development of j industry consensus standards in lieu of State and Federal regulations. During

Intearated Materials Performance Evaluation Proaram Review at Reaion P-A review team comprised of staff from the Office of Nuclear Material Safety

,nd Safeguards (NMSS). the Office of State Programs. Region II. and the State af Tennessee, conducted an Integrated Materials Performance Evaluation Program j (IMPEP) review of Nuclear Regulatory Commission Region IV programs. They evaluated the Walnut Creek (California) Field Office on March 12-14, 1997, and the Arlington, Texas staff on March 17-21, 1997. The team, led by NMSS used a set of common and non-common performance indicators to evaluate the adequacy of Region IV 9 materials, fuel cycle, and uranium mills programs. A draft report will be sent to Region IV in April. A Management Review Board meeting l' will be held later this S] ring to review the preliminary findings, and to make final eterminations on t1e adequacy of the Region IV program.

Meetina with the Deoartment of Enerav on Materials Disoosition Proaram y~ On March 24. 1997 Nuclear Regulatory Commission staff met with the Department of Energy (DOE) to discuss Attachment B Statement of Work for NRC in Support u of DOE Fissile Materials Disposition Program." to the Reimbursable Agreement

? modified to reflect the Record of Decision for the Iturage and disposition of weapons-usable fissile materials issued by DOE in February 1997.

Meetinas on Sranium Recovery Standard Review Plan On March 17. 1997, staff from the Division of Waste Management (DWM) and the Center for Nuclear Waste Regulatory Analyses (CNWRA) met with representatives

, from the Wyoming Department of Environmental Quality (WDEO). The purpose of the meeting was to: (1) discuss technical concerns related to in situ uranium mining: (2) identify ways to streamline and coordinate licensing reviews; and (3) have the WDEO provide early input on the Standard Review Plan (SRP) for in situ leach facility (ISL) applications now under development by DWM.

i On March 19, 1997. DWM, CNWRA staff, representatives of the National Mining l Association, the Wyoming Mining Association, the Uranium Producers of America. I

_ individual licensees, and interested members of the public participated in a meeting to obtain early input from the industry and the public on the content of the proposed ISL SRP. The National Mining Association agreed to provide specific written suggestions.

U.S. I!ranium Conference On March 19 and 21. 1997. staff from the Division of Waste Management, and the

- Center for Nuclear Waste Regulatory Analyses (CNWRA) participated in the U.S.

g~ Uranium Conference held in Riverton, Wyoming. Other organizations represented

l Wyoming. The purpose of the c.onference was to highlight the resurgence of the uranium mining and milling industry in Wyoming and the nation, and to discuss ,

the future of nuclear energy. DWM staff participated on several panels 1 covering cleanup after mining and milling, groundwater restoration at in situ i leach facilities (ISL), and ISL byproduct disposal. In addition. DWM staff made a presentation on " Regulatory Streamlining - Future Perspectives for the Nuclear Regulatory Commission."

Meetino with Reoresentatives of Bear Creek Uranium Comoany i On March 25, 1997. Division of Waste Management (DWM) staff met witn l representatives of Bear Creek Uranium Company to discuss site reclamation activities and the licensee's February 28, 1997, application for alternate concentration limits (ACLs) for groundwater cleanup. A representative from the Wyoming Department of Environmental Quality participated in the meeting by 1 telephone. The licensee discussed details of its ACLs submittal and provided staff with an overview of its plans for construction including radon barrier alacement and testing, and selection and placement of the erosion protection aarrier. In order to meet the date by which the final radon' barrier must be 1 in place, the licensee has established an ambitious schedule predicated on DWM l staff ap3roval of ACLs at the site. The licensee was notified that the ACL I review s1ould be completed by May 15, 1997, assuming the DWM staff does not identify a need for additional information. The May review completion date j would enable the licensee to meet the schedule for completion of the final .

Office for Analysis and Evaluation of Operational Data Item? of Interest Week End' , March 28, 1997 AE00 Soecial Study. S97-01. "0conet Jectrical System Desian and 00eration" AE0D issued the final report for Special Study S97-01, "0conee Electrical System Design and Operation." The AE0D report documents an independent evaluation of the design and operation of the Oconee emergency electrical system primarily based on operating experience. The evaluation was performed '

to assist the Committee to Review Generic Requirements in a review of this system directed by the Executive Director for Operations in August 1995. The efforts were initiated in the fall of 1995 and a draft of the report was issued for peer review by Duke Power Company on July 8, 1996, along with a draft report on the emergency electrical system prepared by NRR.

Since issuance of the draft report, the Oconee station has initiated numerous activities including modifications to hardware and enhancements to operating 1 procedures relating to the emergency power system. In early January 1997, ,

Oconee also took advantage of a three unit shutdown to perform a series of l tests of the emergency power system. The activities as presented to the NRC l by Oconee address the concerns listed in the conclusions of the AEOD report.  !

Satisfactory completion of those activities should resolve those concerns.

Nuclear Material Events Database Trainina Samuel L. Pettijohn in the Nuclear Materials Assessment (NMED) Section l conducted training on March 24, 1997, in Region III on the Nuclear Material Events database (NMED) for 18 members of the Reginn Ill staff in the Division of Nuclear Materials Safety (DNMS).

The NMED is a computer database of material, fuel cycle, and nonpower reactor events reported to NRC by NRC licensees. Agreement States, non-licensee facilities, and the. Miic. Most (about 70%) of the events reported by NRC licensees are requirec by NRC regulations to be reported to NRC by NRC licensees. Equivalently Agreement State licensees are required to report the  !

same type events to Agreement States. Agreement States report these events to NRC under a voluntary exchange of information program.

AE0D developed the NMED under contract with the Idaho National Engineering and Environmental Laboratory (INEEL). AEOD also maintains the database under contract with INEEL. AEOD is working with IRM to provide access to NMED by 1 regional staff via local AUTOS networks.

Region III is the second region to receive training on NMED. Mr. Pettijohn conducted training for eleven members of the Region II staff in the Division of Nuclear Material Safety (DNMS) on 2/20/97.

Incident Resoonse Trainino IRD has completed biennial incident response training for NRR project managers. A total of 57 NRR project managers (about 70 percent of the target MARCH 28, 1997 ENCLOSURE D

audience) and one project director attended. Six 2-hour training sessions were presented between March 11 and March 26, 1997. The course received favorable feedback from the participants.

scored and discussed proposals. Each participant received a course manual l that will serve as a reference tool.

Montoomery County Annual Transoortation Awards Ceremony The Commuter Services Section of che Montgomery County Division of Transit Services has invited the NRC to be an honorary host of its 12th annual Transportation Awards Ceremony breakfast on May 14, 1997. At this special event the County will recognize key business leaders, top agencies, and County employees who foster the use of mass transit and alternative transportation.

OGC and NMSS staff met with USEC officials on March 27. 1997, in a closed meeting to discuss the status of USEC's privatization effort including the NISPOM foreign ownership control or influence determination and the foreign ownership, control or domination statutory determination. l The Division of Security, in coordination with NMSS. OGC and the Department of Energy, provided USEC with comments on its final draft of " Restrictions on {

Foreign Involvement in USEC's Privatization" on March 26, 1997. The comments -

reflected some concerns regarding references to NRC preliminary assessments on l 2

Office of Personnel Items of Interest Week Ending March 28, 1997 Breast Cancer and Mammoaraohv Education Proaram Soonsored by the Headauarters Emoloyee Health Center On March 27. 1997, the Headquarters Employee Health Center sponsored a Breast Cancer and Mammography education program. The presenter was Lynn Turner, chief nc.amography technician for Healthsouth Imaging. She discussed the current. guluelines for mammography and told attendees how to ensure that the 4

laboratory performing their mammogram was certified and that the technician was competent. She also stressed the importance of regular breast self-examination for all women over the age of 18. Healthsouth is offering mammograms at a reduced rate to NRC employees during the month of April. ,

Working Group (IWG) on Environmental Justice (EJ) Subcommittee on Policy and Coordination held in Washington, DC. Most notable of the agenda items was the  !

Justice under the National Environmental Policy Act (NEPA). Brad Campbell. l White House Council on Environmental Quality (CEO) representative in

attendance at the meeting asked agencies to identify any significant concerns  !

The facility still conducts metallurgical production activities, i but no longer uses radioactive material in its operations. Region I staff terminated Texas Instruments' NRC License. ! Florida Power and Liaht Comoacy - St. Lucie On March 27. Region II conducted a management meeting with representatives of l Florida Power and Light Company regarding the St. Lucie plant. The meeting was one in a series of bi-monthly meetings held to discuss the progress in the '

St. Lucie Performance Improvement Program. Also discussed was the status and improvement in'the St. Lucie Speakout employee concerns program.

Southern Nuclear Ooeratina Comoany - Farley The NRR Team Leader, accompanied by a Region II Dep'uty Director.-Division of. t Reactor Safety, presented the results of the Architect Engineering Design  :

Inspection at an exit meeting open to the public. The inspection found that i systems reviewed were capable of performing their intended safety functions and that there were no unresolved operability issues. The inspection did identify several issues requiring followup, including 10-CFR 50.59 evaluations and tornado missile protection for the turbine driven auxiliary feedwater pump stack and the exhaust silencers for diesel generators. Licensee management indicated in the exit that they intended further discussion with the NRC l

regarding these issues. Three members of the local press attended the meeting.
