This report provides infonnation as required by 10CFR50.55(e) with respect to the Novenber 22, 1985, telephone report of a potential significant deficiency.

SUMMARY
Tne control wire insulation on the auxiliary switch on K-1600 and K-2000, as well as K-225, K-600, and K-800 circuit breakers, may be cut or dan-aged by the top edge of the circuit breaker dust shield when the breaker is racked out to the full disconnect position with the conpartuent door closed. This infonnation was identified by a Part 21 report sthmitted to the NRC by Brown Boveri Conpany (BBC) dated March 19, 1985.
It was brought to Duquesne Light Conpany's (DLC) BVPS-2's attention by the NRC Sr. Resident Inspector in Inspection 85-19, Unresolved Iten 85-19-01.
Ef forts are underway to inspect and repair, if necessary, these circuit breakers.

IMMEDIATE ACTION TAKEN On November 22, 1985, Mr. R. J. Wallauer, DLC, Regul atory Af f airs Depart-ment, notified Mr. D. Johnson, NRC Region I Of fice, by telephone that DLC considered this matter to be potentially reportable under 10CFR50.55(e).

DESCRIPTION OF THE DEFICIENCY Tne control wire insulation on the auxiliary switch on K-1600 and K-2000 BBC K-Line CRT breakers may be cut by the top edge of the dust shield when the breaker is racked out to the full disconnect position with the coapartment door closed.
Tne sane condition could occur on saalier size electrically operated K-800, K-600, and K-225 CRT breakers.
This concern was identified in a Part 21 report submitted by BBC dated March 19, 1985, to Mr.
R.
C.
DeYoung, Director, Of fice of I& E, NRC.
Certain of the subject BBC circuit breakers are used at BVPS-2 in both QA Category I and 11 applications.

ANALYSIS OF SAFETY IMPLICATION Danage to the auxiliary switch wires could result in misoperation of the circuit breaker, depending upon the configuration.
No specific instances of serious insulation danage hate been identified to date.
However, if such a condition had gone undetected, it is conservatively assuned that it could have affected the safe operation of the plant.
5.
CORRECTIVE ACTION TO REME0Y DEFICIENCY Ef forts are currently underway to inspect and repair, if necessary, these circuit breakers.
Two corrective actions are in progress at BVPS-2 to resolve the subject problens wi th BBC K-Line circuit breakers.
If d anaged insulation is found on any of the K-Line breakers, heat-shrink insulation will be added to the control wiring to protect and further insulate the wiring.
For the ebject BBC K-Line circuit breakers, an adhesive backed gasket will be installed on the top edge of the dust

These corrective actions are reconmended by BBC and are being impienented and tracked at BVPS-2 via Engineerinj Field Action Report EFAR-681.

tion of the subject BBC K-Line circuit breakers.
