We have received your letter dated February 17,1997, responding to the suspension of your site use permit, #B403. That suspension of authorization to use the commerciallow-level radioactive waste disposal site operated by US Ecology near Richland, Washington, resulted from the violation of specific regulatory requirements and/or conditions of the site j
operator's license. The Reinstatement Committee met on March 17,1997, and approved and accepted your corrections, based upon the point-of-origin inspection on March 10,1997, and on your quality assurance procedures dated February 18,1997 (assuring future shipments will not repeat the violations). A related violation in the fature will result in escalated enforcement action by the state.
You are hereby notified that your authorization, site use permit #B403, to use the commercial low-level radioactive waste disposal site, is reinstated as of 1:00 p.m., PST, on March 17,1997. Be advised that this authorization is contingent upon your adherence to the quality assurance procedure submitted to the Division of Radiation Protection, i
remittance of the point-of-origin inspection fees, and upon full compliance with pertinent federal and state of Washington regulations, and conditions of the site operator's license.
