NRC INSPECTION REPORT 50-482/97-05 AND NOTICE OF VIOLATION

Dear Mr. Maynard:
An NRC inspection was conducted January 27-31 and February 10-14,1997, at your Wolf
'
Creek Generating Station reactor facility. The enclosed report presents the scope and
results of that inspection. The overall results of this inspection were discussed with
~
Mr. T. Damashek of your staff on April 2,1997.
The inspection team concluded that conditions adverse to quality were being appropriately
)
identified at the Wolf Creek Generating Station. Your quality assurance program had
identified a weakness in the ability to perform effective root cause evaluations and
corrective actions for significant performance improvement requests. We noted that you
had initiated significant corrective actions for the identified weakness including the
,
establishment of a Corrective Action Review Board, the selection of Performance
,

Improvement Coordinators, and planned root cause evaluation training. We encourage full
implementation of your corrective actions to improve the performance of your root cause
evaluations and the effectiveness of your corrective action program.

The team also identified that you had performed an acceptable functional assessment of
!
your auxiliary feedwater system as corrective action for NRC Enforcement
Action EA 96-124, dated July 1,1996. The assessment identified several weaknesses,
including inconsistent implementation of (1) lessons-learned from industry experience,
(2) testing and surveillance requirements, and (3) completion of corrective actions for
closed performance improvement requests. A noncited violation associated with a failure
to adequately perform a Technical Specification Surveillance Requirement was identified.

The inspection identified a violation involving the failure to correct an identified low flow
'
condition to the component cooling water system radiation monitor. This violation is cited
in the enclosed Notice of Violation (Notice) and the circumstances surrounding the violation
is described in detailin the enclosed report. Please note that you are required to respond
'
to this letter and should follow the instructions specified in the enclosed Notice when
preparing your response. The NRC will use your response, in part, to determine whether
further enforcement action is necessary to ensure compliance with regulatory
' requirements.
