Dr. J. Nelson Crace SPR 85-010 Page Two August 30, 1985 EVENT DESCRIPTION On August 5, 1985 water from a leaking lavatory was discovered running down a conduit in the ceiling of , the Cable S pr e ad in g Room located on the 448 foot elevation of the Control Build ing.

Subsequent investigation revealed two 4" x 6" blockouts failed to meet the requirements of the Licensee's Fire Pr ot ec t ion Eva lua t ion (FPE). The FPE requires penetrations to be sealed with material which provides a fire-resistance rating equivalent to that of the barrier in which the penetration-is located. The ceiling of the c ab le s pr e ad in g room is rated as a three hour fire barrier.

CAUSE OF EVENT An apparent deviation in construction s eque nc e resulted in the

. fire wall being installed without sealing the blockouts. The blockouts extend from the ceiling of the cable spreading room through the floor of the Control Room and contain electrical conduits. -The e lectrical circuit s terminated in an electrical panel mounted within a fire rated wall. Albi Dura Spray fire retardant coating was applied to the ceiling of the c ab le spreading room which masked the opening. With the fire wall in place and the fire retardant c oa t ing masking the blockout, the discrepancy was not noted on the final inspection and turnover of the areas.

ACTION TAKEN Area smoke detectors were verified o pe r ab l e , a one hour fire watch patrol was established, and a maintenance work request was initiated for inspection of the ceiling to determine the extent of the fire. barrier degradation. On August 8, 1985 the fire barrier was re tu r ned to o pe r ab le statua by sealing the blockouts and restoring the drywall._ A review of design drawings and wa lkdo wn of affected areas revealed no additional unsealed blockouts. The Licensee has determined that this is an isolated case for which no additional corrective action is warranted.
