JAtM 3 E86 ENCLOSURE NOTICE OF VIOLATION Georgia Power Company Docket No. 50-424 Vogtle 1 License No. CPPR-108 The following violation was identified during an inspection conducted on November 1 - December 2, 1985. The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).
10 CFR 50, Appendix B, Criterion V and the accepted QA program (FSAR Section 17) requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
The licensee has implemented these requirements by issuance of the Georgia Power Company Quality Assurance Manual, Revision 8, and the Startup Manual (SUM).
Contra ry to the above, activities affecting quality were not adequately prescribed in that on November 16 and 18, maintenance activities regarding the oreplanning, review and execution of Maintenance Work Orders 1811851, 1811852, and 1811088 associated with the disassembly and reassembly of the Train "A" and B" chemical volume and control centrifugal charging pumps resulted in the following:

Issuance of Maintenance Work Orders without specific detailing of work
- steps, 2.
One maintenance package was incomplete; and when later completed, it included an uncontrolled document, 3.
One hold point was improperly voided, while others were applied in the nonapplicable portions of the procedure, and 4.
QC and craft were working to verbal directions of a vendor representa-tive in contradiction to procedure details.
In summation, work packages did not contain sufficient detail to allow for proper reviews, establishment of hold points, and performance of work in a quality manner as required by Startup Manual Procedure No. 22, Rev. 10, Maintenance Work Order.
l This is a Severity Level IV violation (S'ipplement II).
Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
