l If you are an flRC licensee and conduct activitics under the General i Licenses of Subpart C of 10 CFR Part 71 or an Agreenent State licensee 1

subject to the requirenents of 10 CFR Part 71, as required by 10 CFR Part 150.20, a quality assurance progran approved by the Con.aission as satisfying the provisions of Subpart H of 10 CFD Part 71 is required.

l You should request renewal of your Cuality Assurance (QA) prograrn at i least 30 days prior to the expiration date. This will provide for l continuation of your QA prooran to satisfy part of the provisions of Subpart C of 10 CFD. Part 71 until a final deternination has been taken on your anolication.

Unless an exemption is provided by 10 CFR Part 170.11, an application i fee of $150.00, as required by 10 CFR Part 170.31, should be included with your request for renewal. This notice of expiration should not be
