. Dear Mr. Subalusky: 3 On February 6,1997, the NRC completed an inspection at your LaSalle facility. The enclosed report presents the results of that inspection. j During this eight-week inspection period, your staff's performance was characterized by l , instances of procedural non-compliance, lack of knowledge regarding existing system i 1 i: configu'ations and configuration changes resulting from surveillance testing, and the failure l to self-check. These human performance deficiencies along with configuration control j problems that involved removing the wrong component from service and continuing 1 concerns with the work control process, which were identified during this inspection '

period, indicate that corrective actions in these areas have not been effective to date. f .We are.also concerned with two violations of NRC requirements tint were identified during , this inspection period. The first violation involves four examples of failing to follow i , procedures. The procedures pertain to the removal of Division 3 breakers from service, a l fire protection surveillance test, transfer of liquid radioactive waste, and implementation of l I fire protection administrative work controls. Although two of the examples were licensee- identified, they are included in the cited violation because they stem from continued

a - weaknesses in the work control process and out-of-service program. The second violation i involves a deficient out-of service instruction which resulted in an operator removing the wrong safety-related bettery breaker from service. The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding the violations are described q in detail in the enclosed inspection report. ' . You are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part, . 1 to determine whether further enforcement action is necessary to ensure compliance with / ' regulatory requirements. ,
