Mueller Site Vice President Zion Station Commonwealth Edison Company '101 Shiloh Boulevard Zion,IL 60099 SUBJECT: NRC INSPECTION REPORT 50-295/96-20; 50-304/96-20 AND ' NOTICE OF VIOLATION 4 Dear Mr. Mueller: On February 5,1997, the NRC completed an inspection at your Zion 1 and 2 reactor q , facilities. The enclosed report presents the results of that inspection.
i During this eight-week inspection period, your staff's performance was characterized by procedural non-compliance and the acceptance of degraded or abnormal plant conditions. Operations and engineering personnel failed to recognize and appropriately evaluate the degraded material condition of the Unit 2 containment until prompted by the NRC. In addition, a control rcom operator and maintenance workers did not promptly inform operations management of unexpected changes in the configuration of equipment [ observed during containment spray system maintenance activities. We are also concerned about four violations of NRC requirements that were identified during this inspection period. The first violation involves a failure to follow an abnormal operating procedure for a malfunctioning Eagle 21 process protection rack. The second violation pertains to a failure to follow site procedures governing the control of out-of- service equipment. The third violation involves the failure to determine the cause of water intrusion into the 1 A auxiliary feedwater pump turbine, inboard bet..ing reservoir. The ] fourth violation pertains to the failure to follow design control measures for a design i change implemented during corrective maintenance on Unit 2 power operated relief valves.

surrounding the violations are described in detail in $e enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the l enclosed Notice when propering your response.
