SUBJECT: NRC INSPECTION REPORT 50-458/97-01

Dear Mr. McGaha: '

An NRC inspection was conducted January 27 through February 14,1997, at your River

Bend Station reactor facility. The enclosed report presents the scope and results of that

inspection.

The inspection included an evaluation of an applicant for a senior operating license. It was

determined that the applicant satisfied the requirements, and the appropriate license has

been issued. However, it was noted that the applicant performed marginally on the vyritten

examination. Other inspection results are discussed on page 2 of the enclosed report.

During this inspection while finalizing the draft examination, two potential examination j

compromises occurred which were contrary to examination security practices agreed to by

licensee staff. The two incidents were unrelated and there was no indication of an actual

compromise of the examination. However, corrective actions resulted in changes to the

planned examination and in a delay of part of the examination. The incidents also brought

to the forefront the importance of security measures related to the implementation of the

pilot examination process. The NRC has recently reviewed the pilot examination process

and decided to pursue a change to rulemaking which will require power reactor facility

licensees to participate in the examination development process. The NRC informed

licensees of this decision through Generic Letter 95-06, Supplement 1, " Changes in the

Operator Licensing Program" dated January 31,1997. In order to achieve a successful

omgram, examination security must remain a high priority.

In addition, over 5 percent of the written examination answers were changed based on

technical accuracy issues identified in your postexamination review. In accordance with

supplemental guidance to Generic Letter 95-06, we request that you provide the NRC with

informatiun explaining your staff's f ailure to identify these issues before examination

administration. Also describe corrective actions your staff plan to take to assure that the

next licensed operator written examination is of higher quality. We would request your

response within 30 days.
