If you are an NPC licensee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71 or an Agreement State licensee subiect to the requirements of 10 CFR Part 71, as recuired by 10 CFR Part 150.20, a quality assurance progran approved by the Conmission as satisfying the prnvisions of Subpart H of 10 CFR Part 71 is required.
You should request renewal of your Ouality Assurance (CA) progran at least 30 days prIcr to the expiration date. This will provide for continuation of your DA program to satisfy part of the provisions of Subpart C of 10 CFR Part 71 until a final detemination has been taken on your application.
Unless an exenption is provided by 10 CFR Part 170.11, an application fee of $150.00, as required by 10 CFR Part 170.31, should be included stith your request for renewal . This notice of expiration should not be construed that such notice trill be provided in the future. If you do not desire to renew your QA program, please let ne know.
