NRC INTEGRATED INSPECTION 50-333/97-01 AND NOTICE OF VIOLATION

Dear Mr. Colomb:

On February 22,1997 the NRC completed an inspection at your FitzPatrick Power Plant.

The enclosed report presents the results of that inspection. During the 7-week period

covered by this inspection period, your conduct of activities at the plant were generally

characterized by safe plant operation. A large influx of fish blocked the travelling screens

on January 23,1997, causing a reduction in intake water level and leading to a manual

!

reactor scram. Operator response was appropriate, allowing the plant to be quickly placed

in a safe condition. However, the NRC noted that automatic start function for 2 of the 3

i

travelling screens was disabled at the time of the event. The work planning and control

process did not recognize the potential risk of this evolution, which resulted in removing

j

two travelling screens from service at the same time. Your continued attention to this area

,

is warranted.

The emergency preparedness (EP) program was reviewed during this inspection period,

i

and, overall, was fouad to be directed toward public health and safety. Your radiological

protection program has made significant improvements. However, we note that weak

contractor oversight during the outage led to rework and expansion of the outage scope,

causing increased radiation dose.

)

in the engineering area, a violation of NRC regulations pertaining to the performance of

safety evaluations for modifications to certain safety-related air-operated valves was

identified as outlined in the attached Notice of Violation. You are required to respond to

this letter and should follow the instructions specified in the enclosed Notice when

preparing your response. In your response, you should document the specific actions

taken, and any additional actions you plan, to prevent recurrence.

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosures, and your response will be placed in the NRC Public Document Room (PDR).

The responses directed by this letter and the enclosed Notice are not subject to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
