APPENDIX A NOTICE OF VIOLATION Louisiana Power and Light Company Docket:

50-382 Waterford Unit 3 Permit:

NPF-26 During an NRC inspection conducted on January 7-11, 1985, e violation of NRC requirements was identified. The violation involved failure to have procedures for activities which affect quality.

In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1985), the violation is listed below:

Licensee Failure to Have Procedures to Assure Compliance with Waterford SES Unit 3 Operations Quality Assurance Program, Section 17.2.23, and FSAR Table 3.2.1, Note 12.

10 CFR Part 50, Appendix B, Criterion V requires that activities affecting quality be prescribed by documented instructions and procedures.

1 Waterford SES Unit 3 Operations Quality Assurance Program, Section 17.2.2.3 states:

" Table 3.2-1 of this FSAR provides the safety-related classification of plant structures, systems, and components and identifies those items subject to the 10 CFR 50, Appendix 0, Quality Assurance Program as herein described."

Waterford SES Unit 3 FSAR Table 3.2-1 includes a Note 12 as being applicable for the category " Electric Systems and Equipment."

Note 12 states:

" Electrical Equipment - All cables, relays, motors, switchgear and other electrical equipment serving safety system components required to function during the SSE shall be Class 1E, if they are necessary for the performance of the component's function. All Class 1E electrical equipment will receive applicable portions of 10 CFR 50 Appendix B OA during the operations."

t Contrary to the above, the licensee had no procedures to assure implementation of these requirements and comitments.

Failure to Have Maintenance Procedures for Safety-Related Equipment Which l

Reflect Equipment Manufacture's Recommended or Suggested Maintenance i

10 CFR Part 50, Appendix 0, Criterion V reouires that activities affecting quality shall be prescribed by documented procedures and instructions.

The licensee has conunitted to Regulatory Guide 1.33, Revision 2, and ANSI 18.7-1976(withcomments). ANSIN18.7,Section5.3.5(4), states:

"Where i

appropriate sections of related documents such as vendor manuals or t

equipment maintenance instructions provide adequate instructions to assure i

the required quality of work, the applicable sections of the related documents shall be referenced in the procedure."

Contrary to the above, the licensee failed to have maintenance procedures for Emergency Feed Water (EFW) pump motors which addressed the motor manufacturer's suggested and reconunended maintenance for these motors.

l This is a Severity Level IV violation (Supplemental II.d) (50-302/8501-03).

Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted.

the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further violations, and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
