SUBJECT: NRC EFFLUENTS INSPECTION REPORT 50-213/97-02

Dear Mr. Feigenbaum:

This letter refers to the routine safety inspection conducted by Messrs. J. Jang and l

L. Eckert of this office on February 3-7, and FeL.uary 24-26, 1997, of activities authorized i

by NRC license No. DPR-61 at the Haddam Neck Station, East Haddam, Connecticut and I

the discussion of our findings held with Mr. J. LaPlatney and other members of your staff

at the conclusion of the inspection.

Our inspection focussed on your programs for radioactive liquid and gaseoas effluent

control, including organization and effectiveness of program implementation, radiation

monitoring calibration, air handling system surveillance and monitoring, and implementation

of QA/QC at your chemistry laboratory. While portions of this program remain effective, a

general decline in this area has been noted. One of your activities, involving inadequate

calibration of various detectors in the radiation mordioring system, is considered an

apparent violation, as detailed in Section R 2.1 of this report. In view of previously

identified deficiencies in your radiological controls program (detailed in NRC Inspection ,

Report 50-213/96-12, dated December 19,1996), for which escalated enforcement is l

currently being considered, enforcement action relative to this latest issue will be discussed '

in separate correspondence. While a Notice of Violation is not currently being issued, you

are required to respond to this letter by identifying the results of your corrective actions as

described in your letter dated February 24,1997. Your response should include a

discussion of the results of your root cause evaluation, subsequent corrective actions,

including measures taken or planned relative to the performance of the Plant Operations

Review Committee (PORC) in view of its approval of the deficient HMS calibration

procedures; and Quality Assurance efforts to monitor and ascertain the quality of l

performance in this area. The NRC will use your response, in part, to determine whether i

further enforcement action is necessary to ensure compliance with regulatory

requirements.
