Dear Mr. Thomas:

During a recent NRC inspection of our Reactor Facility in the operations area, it was discovered that several unplanned shutdowns occurring on our 2 MW UVAR reactor, License No. R-66, during the calendar year 1984 had been inadvertently omitted from our 1986 annual report, upon transcription from the reactor logbook. We are therefore submitting an addendum to our 1984 annual report listing all of our unplanned shutdowns and corrective actions taken. It should be pointed out that the shutdown frequency registered is common to research reactors. Generally, the scrams have absolutely no safety implications for the operation of the reactor, resulting from building power failures, electronic noise, etc. In the addendum, the shutdowns that were inadvertently left out are marked Wft a. asterisk.
