APPENDIX A

! National Technical Services l Docket No. 99900914/85-02 NOTICE OF NONCONFORMANCE hsed on the results of an NRC inspection conducted on October 30-31, l November 1,1985, it appears that certain of your activities were not

! conducted in accordance with NRC requirments. Nonconformance with these 1

requirements are as follows:

A. Criterion V of Appendix B to 10 CFR Part 50 states in part: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordence with these instructions, procedures, or drawings."

Paragraph 2.5.1 of the Quality control Manual (QCM), Revision 3, dated August 1,1984, states, in part, "The test engineer is responsible for collecting the necessary specifications, drawings, and test procedures, and including them in the job instruction package."

Paragraph 2.6.1 of the QCM, states, in part, "The procedure shall describe the function measured, the equipment that will be used to measure the function and shall include: schematic diagrams, charts, sample data sheets, or any other information as required to help clarify the procedure."

Contrary to the above, (1) the job instruction package for Master Job Order (MJ0) 558-1711 did not contain a procedure describing the detailed operation of the steam supply system used for qualification testing and (2) the circuit used for monitoring the RTD output signals during the LOCA test was not described in Test Procedure 558-1711. <

8. Criterion V of Appendix B to 10 CFR Part 50 states in part: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

Test Procedure 558-1711 required that measurements during the three-point calibrations should be taken every minute until two consecutive readings differed by 0.02 ohms or less, indicating stabilization.

Contrary to the above, only one measurement was recorded by the test technician on the data sheet for MJO 558-1711.

C. Criterion V of Appendix B to 10 CFR Part 50 states in part: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings."

Paragraph 10.3.2 of the QCM, states, in part, "In the event that the calibration interval shown on a calibration level affixed by an outside calibration laboratory does not correspond with the interval specified in NTS's Official Equipment List, the Quality Control Department shall apply .

an NTS calibration label."

Contrary to the above, NTS calibration label was not affixed to a Gould stripchart recorder when the calibration interval for the outside calibration laboratory was different from the NTS official equipment list calibration interval.

D. Criterion V of Appendix B to 10 CFR Part 50 states in part: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or '

drawings."

Page A-27 of the QCM, states, in part, "The provisions of 10 CFR Part 21 shall be specified by reference on each procurement document for materials and services." ,

Contrary to the above, the provisions of 10 CFR Part 21 were not specified by reference on purchase order 631-054814C.

E. Criterion XVII of Appendix B to 10 CFR Part 50 states in part: " Sufficient records shall be maintained to furnish evidence of activities affecting quality. The records shall include at least the following: Operating logs and the results of reviews, inspections, tests, audits, monitoring of work performance, and materials analyses. The records shall also include closely-related data such as qualifications of personnel, procedures, and equipment. Inspection ard test records shall, as a minimum, identify the inspector or data recorder, the type of observation, the results, the acceptability, and the action taken in connection with any deficiencies noted. Records shall be identifiable and retrievable."

Page A-23 of the QCM, states, in part, " Test records are considered to be evidence of activities pertaining to quality, they shall be legible, identifiable, and retrievable." " Test Records include: a) test data calculations and formulas, inprocess test inspection records and data sheets, and data recorder charts."

Contrary to the above, (1) NTS was unable to identify the source of the activation energy value for ethylene propylene as stated in TP l

558-1711 for MJO 558-1711,(2) there was no documented record in MJO File 558-1711 that showed the chemical spray solution was mixed as

prescribed in the test data calculation, and (3) two blue traces that

! measured temperature parameters during LOCA testing for MJO 558-1711 overlapped on the stripchart preventing clear identification of which trace was which.
