INSPECTION REPORTS 50-254/97003(DRS); 50-265/97003(DRS)

Dear Mr. Kraft:
'
On February 5,1997, the NRC completed an inspection at your Quad Cities Nuclear Power
,
Station. The purpose of the inspection was to review the gaseous and liquid radioactive
release and effluent monitoring program. The enclosed report presents the results of that
inspection.
Although the mataal condition and operability of your radiation monitoring system was
>
generally good, a number of problems were identified in this area during the inspection. In
particular, printer problems or electrical fluctuations caused a control room radiation
'
monitoring system microprocessor to lock up. Plant operators were required to reset the
system each time this occurred. While this did not effect the operability of the system,
this problem was not reported through your problem identification program nor was it
i
communicated to the responsible system engineer. In addition, the inspectors identified
that your staff did not establish a basis for the alarm setpoints for continuous air monitors
'

and that the staff was unable locate the basis for area radiation monitor alarm setpoints
during the inspection.
Two examples of a violation for the failure to follow radiation protection procedures were
.
'
identified during this inspection. The inspectors identified that the labels for the service
water and radwaste effluent monitor check sources were illegible and that a contaminated
area was not properly posted.
The violation is cited 1 the enclosed Notice of Violation (Notice) and the circumstances
i
surrounding it are described in detailin the subject inspection report. Please note that you
are required to respond to this letter and should follow the instructions specified in the
'
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements,
in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter
and its enclosures will be placed in the NRC Public Document Room.
