O APPENDIX B Power Inspection Inc.

Docket No. 99901033/85-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on October 15-17, 1985, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconformances with these requirements are as follcws:

A. Procedure No. PI-A-04 " Procurement of Items, Material and Services,"

dated November 21, 1981 requires that documents necessary for the procurement of equipment and services be prepared, processed and approved by the General Manager with a subsequent review and approval by the Manager of Quality Assurance.

Contrary to the above, it was noted that eddy current testing equipment and calibration services were obtained from Zetec in 1984 and 1985, but procurement documents were not prepared, processed and approved for the purchase of these items (85-01-03).

B. Section 5.4 of Procedure No. PI-A-IV " Quality Assurance Program -

Administrative Policy," dated October 19, 1981 requires that Power Inspection (PI) provide indoctrination and training of personnel performing activities affecting quality, and the Manager of Quality Assurance retains a record of attendance.

Contrary to the above, there was no documented evidence that PI had indoctrinated and trained any personnel since the company was incorporated in 1982 (85-01-04).

C. Section 5.6 of Procedure No. PI-A-04 states, in part, "Each supplier

...will be evaluated by surveys, audits and a review of past performances."

Section 5.3.1 of Procedure No. PI-A-12 " Audits" dated December 2, 1981 states, in part, "The Manager of Quality Assurance will perform an implementation audit on the Quality Assurance Program once every 12 months."

s Contrary to the above, it was noted that from 1982 to the present, that Zetec, a supplier of eddy current testing equipment and calibration services, was never surveyed or audited by PI; and internal audits of the QA Program have never been performed by PI (85-01-05).

D. Procedure No. PI-A-06 " Control of Measuring and Test Equipment," dated December 1,1981 requires that measuring and test equipment (M&TE) used by PI personnel be calibrated at specified intervals, a Master Index of M&TE (Form No. 2026) be maintained, and a M&TE Data Sheet (Form No. 2027) be completed for each item of M&TE that requires calibration and/or certification.

Contrary to the above, it was noted that eddy current instruments (2),

magnetic tape recorders (2), strip chart recorders (4), vector analyzer (1), and M-17 mixers (2) were never calibrated in 1985; and Form Nos.

2026 and 2027 did not exist for any of the items requiring calibration (85-01-06).

E. Ten Duquesne Light Company (DLC) P0s and nine Consumer Power Company (CPC)

P0s for eddy current testing required that PI's nondestructive exami-nation (NDE) personnel be certified to SNT-TC-1A (June 1975 Edition).

Section 6.0 of Procedure No. PI-A-07 " Certification of NDT Personnel,"

dated December 1, 1981 and Section 9.6.1 of SNT-TC-1A (June 1975 Edition) require that each individual's certification records contain, among other things, the following:

Statement indicating satisfactory completion of training in accordance with PI procedure.

Results of the physical examination.

Copies of current examinations.

Contrary to the above, a review of NDE records for one-Level III, three-Level II, and five-Level I examiners revealed (85-01-07):

1. The records for all nine examiners did not contain a statement showing completion of training in accordance with PI Procedure No. PI-A-07.
2. There was no eye examination for 1982, and it was overdue for 1985 for the Level III. One Level II was missing eye exami-nations for 1984 and 1985, and a 1985 examination was overdue for another Level II.
3. Copies of examinations given in 1983 for the Level III and in 1985 for one Level II were missing.

F. Section 9.7.3 of SNT-TC-1A requires that PI's written practice establish rules covering the duration of interrupted services which will require re-examination and re-certification.

Contrary to the above, it was noted that PI Procedure No. PI-A-07

" Certification of NDT Personnel" did not address the area of duration of _ interrupted services requiring re-examination and re-certification (85-01-08).
