SUBJECT: .NRC INSPECTION REPORT 50-445/97-04;50-446/97-04 AND NOTICE OF
VIOLATION
Dear Mr. Terry:
An NRC inspection was conducted February 10-14,1997, at your Comanche Peak Steam
Electric Station, Units 1 and 2 reactor facilities. The enclosed report presents the scope
and results of that inspection.
During this inspection, one violation of NRC requirements was identified by our inspectors.
The violation involved the failure to correct a previously identified exercise weakness
stemming from implementation of site evacuation procedures. This violation is of concern
because it involves protection of plant personnel and because it indicates that corrective
actions for key emergency responsibilities were not fully effective.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described in detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with

regulatory requirements.

In addition to the violation, one exercise weakness was identified during the simulator
walkthroughs as discussed in Section P4 of the enclosed inspection report. The weakness
involved the failure to make a timely protective action recommendation following a general
emergency declaration.
According to 10 CFR Part 50, Appendix E.IV.F.2.g, exercise weaknesses are inspection
findings that need to be corrected by the licensee. You are requested to provide this office
with a description of your corrective measures and your schedule for completing these
actions within 30 days after receipt of this letter.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosure (s), and your response will be placed in the NRC Public Document Room. To
the extent possible, your response should not include any personal privacy, proprietary, or
safeguards information so that it can be placed in the Public Document Room without
redaction,
