APPENDIX A Westinghouse Electric Corporation Docket No. 99900404/85-02 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on August 26-30, 1985, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Section 5 of Risk Assessment Technology Instruction / Guidance NS-RAT-IG-9, Revision 0, states:
"The goal of a plan for resolution is to either refer the issue to the WRD Safety Review Committee or to write and file an S.E.
letter documenting the basis for a decision not to refer the issue to the WRD Safety Review Committee. The Manager of S.E. will maintain a Safety File containing appropriate documentation of those decisions."
Contrary to the above, the decision not to refer a potential safety issue relating to an error in an analytical procedure for the dropped rod analysis was not documented and filed. The error affected those calculations performed from June 1983 until its discovery in February 1985. The problem was in the direction in which the turbine runback setpoint error was applied in the calculation.
Thus, it only affected plants that utilized turbine runback on dropped rod and for which Westinghouse had done the calculation.
